# Xylometazoline
## Overview
- **Classification**: Alpha-adrenergic agonist, Nasal Decongestant (sympathomimetic)
- **Mechanism**: Acts directly on alpha-adrenergic receptors in the nasal mucosa, causing vasoconstriction of arterioles, leading to decreased blood flow and reduced swelling of nasal membranes.
## Primary Indications
1. **Nasal Congestion** - Due to common cold, sinusitis, or allergic rhinitis.
2. **Facilitate Rhinoscopy** - To shrink nasal mucosa before examination.
3. **Adjuvant in Otitis Media** - To improve Eustachian tube patency.
## Adult Dosing
### Standard Dosing
**Nasal Congestion**
- **Dose**: **1-2 sprays** or **2-4 drops** of **0.1%** solution into each nostril.
- **Frequency**: Every 8-10 hours, up to **3 times daily**.
- **Route**: Intranasal.
- **Duration**: Do not use for more than **3-5 consecutive days**.
### Dose Adjustments
- **Renal Impairment**: Not applicable for topical nasal administration.
- **Hepatic Impairment**: Not applicable for topical nasal administration.
- **Elderly Patients**: Use with caution due to potential for systemic absorption and increased sensitivity to sympathomimetics; monitor for cardiovascular effects.
## Pediatric Dosing
### Neonates (0-28 days)
- **Dose**: **Contraindicated**.
- **Special Notes**: Risk of systemic absorption leading to serious adverse effects (e.g., CNS depression, bradycardia, coma).
### Infants (1-12 months)
- **Dose**: **Contraindicated**.
- **Special Notes**: Risk of systemic absorption leading to serious adverse effects.
### Children (2-11 years)
- **Indication**: Nasal Congestion
- **Dose**: **1 spray** of **0.05%** solution into each nostril.
- **Frequency**: 1-3 times daily.
- **Maximum**: Do not exceed **3 doses** per day.
- **Duration**: Do not use for more than **3-5 consecutive days**.
- **Special Notes**: Not recommended for children <2 years old.
Use pediatric specific formulation (**0.05%** solution).
### Adolescents (12-18 years)
- **Indication**: Nasal Congestion
- **Dose**: **1-2 sprays** or **2-4 drops** of **0.1%** solution into each nostril.
- **Frequency**: Every 8-10 hours, up to **3 times daily**.
- **Maximum**: Do not exceed **3 doses** per day.
- **Duration**: Do not use for more than **3-5 consecutive days**.
- **Special Notes**: Use adult strength (**0.1%** solution).
## Safety Information
### Contraindications
- **Absolute**: Transsphenoidal hypophysectomy or other surgery exposing dura mater.
- **Absolute**: Narrow-angle glaucoma.
- **Absolute**: Rhinitis sicca (dry nasal mucosa).
- **Absolute**: Concomitant use with MAOIs or within 14 days of MAOI discontinuation.
- **Absolute**: Children under **2 years** of age.
### Common Adverse Effects
- **Common (1-10%)**: Nasal burning or stinging, dryness of nasal mucosa.
- **Common (1-10%)**: Sneezing, local irritation.
- **Serious but Rare**: Rebound congestion (rhinitis medicamentosa) with prolonged use.
- **Serious but Rare**: Systemic effects with excessive use (e.g., headache, palpitation, hypertension, tachycardia, insomnia, anxiety), especially in children.
### Key Drug Interactions
- **Monoamine Oxidase Inhibitors (MAOIs)**: Increased risk of hypertensive crisis.
- **Tricyclic Antidepressants (TCAs)**: Potentiates the pressor effects, increasing risk of hypertension.
- **Other Sympathomimetics**: Increased risk of systemic adverse effects (e.g., cardiovascular stimulation).
## Monitoring & Follow-up
- **Before Treatment**: Assess for contraindications (e.g., recent MAOI use, specific surgeries).
- **During Treatment**: Monitor for symptom improvement and duration of use.
- **Clinical Signs**: Watch for signs of rebound congestion (worsening congestion after drug wears off), nasal irritation, or systemic effects (e.g., dizziness, palpitations).
## Clinical Pearls
- 💡 **Limit Duration**: Emphasize use for no more than **3-5 days** to prevent rebound congestion (rhinitis medicamentosa).
- 💡 **Proper Technique**: Instruct patients on correct intranasal administration to maximize efficacy and minimize systemic absorption.
- 💡 **Pediatric Caution**: Ensure correct concentration and age-appropriate dosing in children to avoid overdose and systemic toxicity.
- 💡 **Avoid Sharing**: Do not share nasal sprays to prevent the spread of infections.
- 💡 **First-line alternative**: Consider saline nasal sprays or washes for less severe congestion, especially for long-term use.
