# Walethmade Bromide
## Overview
Walethmade bromide is a quaternary ammonium compound with muscarinic antagonist properties.
## Primary Indications
* Adjunctive treatment of peptic ulcers.
* Management of functional gastrointestinal disorders, such as irritable bowel syndrome, characterized by smooth muscle spasm.
## Adult Dosing
* **Peptic Ulcers:** 15-30 mg orally three times daily, 30 minutes before meals and at bedtime. Maximum daily dose not explicitly defined, but typically ranges up to 120 mg/day.
* **Functional GI Disorders:** 10-20 mg orally three times daily.
## Pediatric Dosing
* Dosing in pediatric populations is not well-established. Safety and efficacy have not been determined.
## Dose Adjustments
* **Renal Impairment:** No specific dose adjustments are established. Caution is advised as the bromide ion may accumulate.
* **Hepatic Impairment:** No specific dose adjustments are established. Use with caution.
## Contraindications
* Narrow-angle glaucoma.
* Achalasia.
* Paralytic ileus.
* Severe ulcerative colitis.
* Tachycardia associated with cardiac disease.
* Known hypersensitivity to Walethmade bromide or other anticholinergics.
## Adverse Effects
* Dry mouth, blurred vision, dilated pupils (mydriasis), photophobia, urinary retention, constipation, decreased sweating (anhidrosis), drowsiness, dizziness, and confusion.
* Tachycardia, palpitations.
* Paradoxical excitement in children.
## Key Drug Interactions
* **Antacids:** May decrease absorption of Walethmade bromide. Administer antacids at least 1 hour apart from Walethmade bromide.
* **Other Anticholinergics (e.g., tricyclic antidepressants, antihistamines, amantadine):** Additive anticholinergic effects may increase the risk of adverse effects.
* **Opioids:** May potentiate paralytic ileus.
* **Beta-blockers:** May increase the effect of beta-blockers on heart rate.
## Monitoring
* Monitor for signs and symptoms of anticholinergic adverse effects.
* Assess for effectiveness in symptom relief.
* Consider electrolyte monitoring, particularly bromide levels, in patients with renal impairment or suspected accumulation.
## Clinical Pearls
* Walethmade bromide is generally less potent than some other anticholinergic agents.
* It is important to counsel patients on potential anticholinergic side effects, especially blurred vision, which can impair driving and operating machinery.
* Due to the bromide ion, caution is warranted with prolonged use, particularly in patients with renal insufficiency, to avoid bromide intoxication (bromism).
