# Walethmade bromide
## Overview
Walethmade bromide is a quaternary ammonium compound with anticholinergic properties. It is poorly absorbed systemically, leading to primarily local effects in the gastrointestinal tract.
## Primary Indications
* Symptomatic treatment of irritable bowel syndrome (IBS) and other functional gastrointestinal disorders, particularly those characterized by abdominal pain and cramping.
## Adult Dosing
* **Recommended Dose:** 20 mg orally three times daily, 30 minutes before meals.
* **Maximum Dose:** Not clearly established in literature, but typically adherence to the recommended TID dosing is sufficient.
## Pediatric Dosing
* Dosing for pediatric patients is not established. Use in children is generally not recommended.
## Dose Adjustments
* **Renal Impairment:** No specific dose adjustments are recommended due to minimal systemic absorption.
* **Hepatic Impairment:** No specific dose adjustments are recommended.
* **Elderly:** No specific dose adjustments are typically required.
## Contraindications
* Hypersensitivity to walethmade bromide.
* Conditions where anticholinergic effects are undesirable, such as paralytic ileus, intestinal atony, severe ulcerative colitis, toxic megacolon, or prostatic hypertrophy.
* Myasthenia gravis.
* Glaucoma (particularly angle-closure glaucoma).
## Adverse Effects
* **Common:** Dry mouth, blurred vision, constipation, urinary retention, drowsiness, dizziness.
* **Less Common:** Nausea, vomiting, abdominal discomfort, headache, nervousness.
* **Rare:** Tachycardia, confusion.
## Key Drug Interactions
* **Amantadine, quinidine, tricyclic antidepressants, MAO inhibitors:** Increased anticholinergic effects.
* **Corticosteroids:** Increased risk of increased intraocular pressure.
* **Antacids and antidiarrheal agents (especially those containing kaolin-pectin):** May decrease the absorption of walethmade bromide; administer at least 1-2 hours apart.
* **Cisapride, metoclopramide:** May antagonize the effects of walethmade bromide on GI motility.
## Monitoring
* Monitor for signs and symptoms of anticholinergic side effects (dry mouth, blurred vision, urinary retention, constipation).
* Evaluate patient's response in terms of relief of abdominal pain and cramping.
## Clinical Pearls
* Administer 30 minutes before meals to maximize efficacy.
* The anticholinergic effects are primarily local, minimizing systemic adverse reactions compared to other anticholinergics.
* Patients with glaucoma or prostatic hypertrophy should be advised to use with caution.
