# WALETHMADE BROMIDE
## Overview
Walethmade bromide is a quaternary ammonium compound that acts as a peripheral anticholinergic agent. It has smooth muscle relaxant properties.
## Primary Indications
* Management of urinary incontinence associated with detrusor overactivity.
* Treatment of motion sickness.
## Adult Dosing
* **Urinary Incontinence:** 50 mg orally 3-4 times daily. Maximum dose: 200 mg daily.
* **Motion Sickness:** 50 mg orally 30-60 minutes before travel, may repeat dose every 4-6 hours as needed.
## Pediatric Dosing
* Dosing in pediatric patients is not well-established. Use with caution and individualize therapy.
## Dose Adjustments
* No specific dose adjustments are recommended for renal or hepatic impairment. However, caution is advised in patients with significant organ dysfunction.
## Contraindications
* Known hypersensitivity to walethmade bromide.
* Conditions where anticholinergic effects are detrimental, such as:
* Glaucoma (narrow-angle)
* Myasthenia gravis
* Obstructive uropathy (e.g., bladder neck obstruction)
* Obstructive gastrointestinal disease (e.g., pyloric stenosis, achalasia)
* Uncontrolled cardiovascular disease
* Thyrotoxicosis
## Adverse Effects
Common adverse effects are anticholinergic in nature and include: dry mouth, blurred vision, constipation, urinary retention, drowsiness, dizziness, and confusion. Less common effects include tachycardia, rash, and excitement.
## Key Drug Interactions
* **Other Anticholinergics (e.g., atropine, scopolamine, antihistamines):** Increased risk of additive anticholinergic adverse effects.
* **Cimetidine:** May reduce the metabolism of walethmade bromide, potentially increasing its plasma concentrations and risk of adverse effects.
* **Opioid Analgesics:** Increased risk of severe constipation and gastrointestinal stasis.
* **MAO Inhibitors:** May potentiate anticholinergic effects.
## Monitoring
* Monitor for efficacy in symptom control (e.g., frequency of incontinence episodes, severity of motion sickness).
* Monitor for signs and symptoms of anticholinergic adverse effects (e.g., dry mouth, constipation, urinary retention, blurred vision, confusion).
## Clinical Pearls
* Walethmade bromide should be taken with a full glass of water.
* Patients should be advised to avoid activities requiring mental alertness or coordination until they know how the drug affects them due to potential drowsiness and dizziness.
* Caution is recommended in elderly patients due to increased sensitivity to anticholinergic effects.
