# Aclidinium Bromide
## Overview
Aclidinium bromide is a long-acting muscarinic antagonist (LAMA) that functions as a bronchodilator. It acts by inhibiting M3 receptors on airway smooth muscle, leading to relaxation. It is structurally unrelated to "Walethmade" and is marketed under the brand name Tudorza Pressair.
## Primary Indications
Maintenance treatment of chronic obstructive pulmonary disease (COPD), including chronic bronchitis and emphysema. Not indicated for the relief of acute bronchospasm.
## Adult Dosing
The recommended dose is 400 mcg administered by oral inhalation twice daily (every 12 hours). Do not exceed 400 mcg per dose.
## Pediatric Dosing
Safety and efficacy have not been established in pediatric patients. The condition (COPD) is generally not applicable to this population.
## Dose Adjustments
No dosage adjustment is required for patients with renal impairment or hepatic impairment.
## Contraindications
Hypersensitivity to aclidinium bromide, atropine, or derivatives. Use with caution in patients with narrow-angle glaucoma, prostatic hyperplasia, or bladder-neck obstruction.
## Adverse Effects
Common: Headache, nasopharyngitis, cough, sinusitis.
Serious: Paradoxical bronchospasm (discontinue immediately), anticholinergic effects (worsening of urinary retention, constipation, or glaucoma).
## Key Drug Interactions
Avoid concomitant use with other anticholinergic medications (e.g., ipratropium, tiotropium) due to the risk of additive anticholinergic systemic effects.
## Monitoring
Monitor respiratory status, including FEV1, dyspnea scores, and frequency of rescue inhaler use. Assess for signs of urinary retention or ocular complications if pre-existing conditions are present.
## Clinical Pearls
- **Administration:** Ensure the patient is trained on the use of the Pressair multidose dry powder inhaler; the device includes a green-to-red control window to confirm successful inhalation.
- **Acute Use:** Emphasize that aclidinium is for maintenance only and does not replace short-acting beta-agonists (e.g., albuterol) for rescue therapy.
- **Anticholinergic Profile:** While systemic absorption is low, systemic anticholinergic side effects can occur, particularly in sensitive elderly populations.
