# Aclidinium Bromide

## Overview
Aclidinium bromide is a long-acting muscarinic antagonist (LAMA) that exerts bronchodilatory effects by inhibiting M3 receptors in airway smooth muscle. It is delivered via a multidose dry powder inhaler (DPI).
## Primary Indications
Maintenance treatment of airflow obstruction in patients with chronic obstructive pulmonary disease (COPD), including chronic bronchitis and emphysema.
## Adult Dosing
The recommended dose is 400 mcg (one inhalation) twice daily.
## Pediatric Dosing
Safety and efficacy have not been established in patients younger than 18 years; therefore, use in the pediatric population is not recommended.
## Dose Adjustments
No specific dosage adjustments are required for elderly patients or patients with renal or hepatic impairment.
## Contraindications
Hypersensitivity to aclidinium bromide, atropine, or derivatives thereof. It is contraindicated as a rescue medication for the relief of acute bronchospasm.
## Adverse Effects
Common side effects include headache, nasopharyngitis, cough, sinusitis, and diarrhea. Serious but rare effects include paradoxical bronchospasm, hypersensitivity reactions, and urinary retention secondary to anticholinergic effects.
## Key Drug Interactions
Avoid concurrent use with other anticholinergic-containing medications (e.g., tiotropium, ipratropium) as this may increase the risk of systemic anticholinergic adverse effects (e.g., dry mouth, urinary retention, narrow-angle glaucoma).
## Monitoring
Monitor for signs of paradoxical bronchospasm (if occurring, discontinue immediately). Monitor for worsening urinary symptoms (prostatic hyperplasia/bladder-neck obstruction) or worsening of narrow-angle glaucoma. Assess lung function (FEV1) to evaluate therapeutic response.
## Clinical Pearls
* **Technique:** Ensure the patient is trained on the use of the specific DPI device to ensure proper medication delivery.
* **Not a Rescue Inhaler:** Must emphasize that this medication does not provide immediate relief from acute COPD symptoms.
* **Storage:** Keep the inhaler closed until the moment of use to prevent moisture contamination of the powder.
