# Aclidinium Bromide (Often misspelled as "Walethmade")
## Overview
Aclidinium bromide is a long-acting muscarinic antagonist (LAMA) that acts as a bronchodilator by inhibiting M3 receptors in the bronchial smooth muscle. It is delivered via a multidose dry powder inhaler (DPI).
## Primary Indications
Maintenance treatment of airflow obstruction in patients with chronic obstructive pulmonary disease (COPD), including chronic bronchitis and emphysema.
## Adult Dosing
400 mcg (one inhalation) twice daily. Do not exceed 400 mcg every 12 hours.
## Pediatric Dosing
Not indicated for use in the pediatric population. Safety and efficacy have not been established.
## Dose Adjustments
No specific dosage adjustments are required for renal or hepatic impairment.
## Contraindications
Hypersensitivity to aclidinium bromide, atropine, or derivatives thereof. Not indicated for the relief of acute bronchospasm (rescue therapy).
## Adverse Effects
* **Common:** Headache, nasopharyngitis, cough.
* **Serious:** Paradoxical bronchospasm, hypersensitivity reactions (angioedema, urticaria), urinary retention, and worsening of narrow-angle glaucoma.
## Key Drug Interactions
Avoid concomitant use with other anticholinergic agents (e.g., ipratropium, tiotropium) due to the cumulative risk of systemic anticholinergic effects and toxicity.
## Monitoring
* Monitor for signs of paradoxical bronchospasm after the first dose.
* Monitor for symptoms of urinary retention or worsening narrow-angle glaucoma.
* Periodic assessment of COPD symptoms and lung function (if applicable).
## Clinical Pearls
* **Technique:** The device includes a "green-to-red" control window to confirm successful inhalation. Patients should hear a "click" indicating the dose is ready.
* **Rescue Inhalers:** Ensure the patient understands that this is a maintenance medication and does not replace short-acting beta-agonists (SABA) for acute attacks.
* **Anticholinergic Burden:** Use with caution in patients with prostatic hyperplasia, bladder-neck obstruction, or narrow-angle glaucoma.
