# Aclidinium Bromide
*(Note: "Walethmade" is not a recognized generic medication; your request aligns with Aclidinium bromide, a long-acting muscarinic antagonist (LAMA).)*
## Overview
Aclidinium bromide is a long-acting anticholinergic (LAMA) that acts as a selective muscarinic M3 receptor antagonist, inducing bronchodilation by inhibiting acetylcholine at smooth muscle sites in the airway.
## Primary Indications
Maintenance treatment of chronic obstructive pulmonary disease (COPD), including chronic bronchitis and emphysema.
## Adult Dosing
400 mcg inhaled twice daily via dry powder inhaler (DPI). Do not exceed 400 mcg every 12 hours.
## Pediatric Dosing
Not indicated for use in pediatric patients. Safety and efficacy in patients under 18 years of age have not been established.
## Dose Adjustments
No dosage adjustment is required for geriatric patients or patients with renal or hepatic impairment.
## Contraindications
* Hypersensitivity to aclidinium bromide, atropine, or derivatives.
* Not indicated for the relief of acute bronchospasm (rescue therapy).
## Adverse Effects
* **Common:** Headache, nasopharyngitis, cough, sinusitis.
* **Serious:** Paradoxical bronchospasm (discontinue immediately), worsening of urinary retention, acute narrow-angle glaucoma, and anticholinergic systemic effects (e.g., dry mouth, blurred vision, tachycardia).
## Key Drug Interactions
Avoid concomitant use with other anticholinergic agents (e.g., ipratropium, tiotropium) due to the risk of additive anticholinergic toxicity.
## Monitoring
* Monitor for signs of paradoxical bronchospasm immediately after administration.
* Monitor COPD symptoms for clinical efficacy.
* Monitor for increased intraocular pressure or difficulty urinating in predisposed patients.
## Clinical Pearls
* **Use instruction:** Ensure the patient understands the DPI mechanism; the device provides visual and auditory feedback (a "click") confirming the dose has been inhaled properly.
* **Rescue therapy:** Instruct patients that this medication is for maintenance only and must **not** be used for acute exacerbations.
* **Storage:** Keep in the sealed pouch until use to protect from moisture.
