# Tamsulosin (Urimax)
## Overview
Tamsulosin is an alpha-1 adrenergic receptor antagonist.
## Primary Indications
* Benign prostatic hyperplasia (BPH): To improve urine flow and relieve symptoms.
* Ureteral stones: To facilitate passage of stones (off-label in some regions).
## Adult Dosing
* **BPH:** 0.4 mg orally once daily.
* Patients not responding adequately may be titrated to 0.8 mg orally once daily.
* **Ureteral stones:** 0.4 mg orally once daily (ensure adequate hydration).
## Pediatric Dosing
Tamsulosin is not indicated for use in pediatric patients.
## Dose Adjustments
No specific dose adjustment is required for patients with hepatic or renal impairment, though caution is advised in severe hepatic impairment.
## Contraindications
* Known hypersensitivity to tamsulosin or any component of the formulation.
* History of postural hypotension.
## Adverse Effects
* Dizziness
* Abnormal ejaculation (including retrograde ejaculation and ejaculatory duct obstruction)
* Headache
* Orthostatic hypotension
* Nasal congestion
* Blurred vision
## Key Drug Interactions
* **CYP3A4 inhibitors (e.g., ketoconazole, ritonavir):** May increase tamsulosin exposure; monitor for adverse effects.
* **Other alpha-blockers:** Additive hypotensive effects.
* **Antihypertensives:** Increased risk of hypotension.
## Monitoring
* Blood pressure, particularly postural changes.
* BPH symptom improvement.
* For patients on concomitant CYP3A4 inhibitors, monitor for tamsulosin-related adverse effects.
## Clinical Pearls
* Tamsulosin exhibits alpha-1a receptor selectivity, which may lead to fewer cardiovascular side effects compared to non-selective alpha-blockers.
* Intraoperative Floppy Iris Syndrome (IFIS) has been observed during cataract surgery in patients taking tamsulosin. Discontinue tamsulosin at least 2 weeks before planned cataract surgery.
* May cause dizziness; advise patients to use caution when driving or operating machinery.
* For BPH, may take several weeks to see full therapeutic effect.
