# Tamsulosin
## Overview
Tamsulosin is an alpha-1 adrenergic antagonist that relaxes the muscles in the prostate and bladder neck, improving urine flow and reducing symptoms of benign prostatic hyperplasia (BPH).
## Primary Indications
* $\text{Symptomatic treatment of benign prostatic hyperplasia (BPH)}$
## Adult Dosing
* $\text{For BPH: } 0.4 \text{ mg orally once daily.}$
* $\text{Maximum dose: } 0.8 \text{ mg once daily.}$
* Dosing should be taken approximately 30 minutes after the same meal each day.
## Pediatric Dosing
* Tamsulosin is not indicated for use in pediatric patients. Its efficacy and safety have not been established.
## Dose Adjustments
* No dose adjustment is typically required for patients with renal impairment.
* No dose adjustment is typically required for patients with hepatic impairment, but caution is advised in severe hepatic impairment.
## Contraindications
* Hypersensitivity to tamsulosin or any component of the formulation.
* History of angioedema.
* Concurrent use with other alpha-1 adrenergic antagonists.
## Adverse Effects
Common adverse effects include:
* Dizziness
* Headache
* Nasal congestion
* Abnormal ejaculation (e.g., retrograde ejaculation, ejaculatory failure)
* Orthostatic hypotension, particularly upon initiating therapy.
## Key Drug Interactions
* **Other alpha-1 adrenergic antagonists (e.g., terazosin, doxazosin):** Increased risk of hypotension. Concurrent use is generally not recommended.
* **CYP3A4 inhibitors (e.g., ketoconazole, ritonavir):** May increase tamsulosin exposure; monitor for adverse effects.
* **Antihypertensives:** Increased risk of hypotension.
## Monitoring
* Monitor for symptoms of BPH.
* Monitor for signs of orthostatic hypotension (e.g., dizziness upon standing).
* Monitor for adverse effects, particularly abnormal ejaculation.
## Clinical Pearls
* Take consistently with the same meal to ensure predictable absorption.
* Advise patients that dizziness and orthostatic hypotension can occur, especially when starting therapy or increasing the dose. Advise to stand slowly from a sitting or lying position.
* Inform patients about the potential for abnormal ejaculation.
* Consider discontinuing tamsulosin prior to cataract surgery in patients who are or will be undergoing such surgery, as it may lead to $\text{intraoperative floppy iris syndrome (IFIS)}$.
