# Urimax (Tamsulosin)
## Overview
Tamsulosin is a selective α1-adrenoceptor antagonist that preferentially blocks alpha-1A receptors in the prostate and bladder neck, promoting smooth muscle relaxation and improving urinary flow.
## Primary Indications
* Treatment of signs and symptoms of benign prostatic hyperplasia (BPH).
* Off-label: Medical expulsive therapy for ureteral calculi.
## Adult Dosing
* **BPH:** 0.4 mg orally once daily, taken 30 minutes after the same meal each day.
* **Maximum Dose:** 0.8 mg daily if no response after 2–4 weeks at 0.4 mg.
## Pediatric Dosing
* **Established:** Not FDA-approved for pediatric use.
* **Off-label/Specialist use:** Sometimes used in pediatric urology for dysfunctional voiding or neurogenic bladder under specialist supervision; dosing is highly variable based on institutional protocol (typically significantly lower than adult doses). **Consult pediatric urology guidelines.**
## Dose Adjustments
* **Renal Impairment:** No dose adjustment required.
* **Hepatic Impairment:** Use with caution in severe impairment; no formal dosing guidelines exist, but monitor for systemic hypotension.
* **CYP3A4/2D6 Inhibitors:** Avoid or exercise extreme caution when co-administering with potent inhibitors.
## Contraindications
* Hypersensitivity to tamsulosin or any component of the formulation.
* History of angioedema.
## Adverse Effects
* **Common:** Dizziness, headache, abnormal ejaculation (retrograde ejaculation), rhinitis, asthenia.
* **Serious:** Orthostatic hypotension (risk highest with first dose), syncope, Intraoperative Floppy Iris Syndrome (IFIS) during cataract surgery.
## Key Drug Interactions
* **Potent CYP3A4 Inhibitors (e.g., ketoconazole, ritonavir):** May significantly increase tamsulosin exposure.
* **Other Alpha-Blockers/Antihypertensives:** Increased risk of severe hypotension when combined with PDE5 inhibitors (e.g., sildenafil) or other antihypertensives.
* **CYP2D6 Inhibitors (e.g., paroxetine):** May moderately increase tamsulosin exposure.
## Monitoring
* Monitor for orthostatic vital signs, especially during titration.
* Monitor urinary symptom improvement (e.g., IPSS scores).
* Assess for PSA changes if screening for prostate cancer.
* Advise patients to inform ophthalmologists of tamsulosin use before scheduled cataract surgery.
## Clinical Pearls
* **Administration:** Must be taken 30 minutes after the same meal each day to ensure consistent bioavailability; taking it on an empty stomach increases the risk of hypotension.
* **IFIS:** Tamsulosin is the most frequent cause of IFIS; surgery should be postponed or managed by ophthalmologists aware of the history.
* **Efficacy:** Maximal benefits on urinary flow are typically seen within 1–2 weeks.
