# Urimax (Tamsulosin)
## Overview
Tamsulosin is a selective α1A-adrenoreceptor antagonist that relaxes smooth muscle in the prostate and bladder neck, improving urinary flow.
## Primary Indications
* Benign Prostatic Hyperplasia (BPH)
* Off-label: Medical Expulsive Therapy (MET) for distal ureteral stones
## Adult Dosing
* **BPH:** 0.4 mg orally once daily, taken 30 minutes after the same meal each day.
* **Dose Titration:** If no response after 2–4 weeks, the dose may be increased to 0.8 mg daily.
* **Maximum Dose:** 0.8 mg daily.
## Pediatric Dosing
* **Safety/Efficacy:** Not established/FDA-approved for pediatric use. Dosage and safety in children are unknown; use is typically restricted to specialized urologic settings under strict protocols.
## Dose Adjustments
* **Renal Impairment:** No dosage adjustment necessary.
* **Hepatic Impairment:** Use caution in severe hepatic impairment (Child-Pugh Class C). Data on dosing in severe hepatic disease is limited; consult specialty guidelines.
## Contraindications
* Hypersensitivity to tamsulosin or any component of the formulation.

* **Ongoing:** Monitor for symptomatic improvement (e.g., I-PSS score) and signs of orthostatic hypotension.
* **Surgical:** Inform ophthalmologist of current or prior use before scheduling cataract or glaucoma surgery.
## Clinical Pearls
* **Administration:** Must be taken 30 minutes after the same meal daily to ensure consistent pharmacokinetics. Do not crush or chew capsules.
* **Fall Risk:** Educate elderly patients on the risk of dizziness/syncope upon standing.
* **IFIS:** Tamsulosin can lead to IFIS during cataract surgery; it is generally recommended to withhold the drug prior to surgery, though the duration of benefit remains debated.
