# Urimax (Tamsulosin)
## Overview
Tamsulosin is a selective alpha-1 adrenergic receptor antagonist that preferentially blocks alpha-1A receptors in the prostate and bladder neck, promoting smooth muscle relaxation and improving urinary flow.
## Primary Indications
* Treatment of signs and symptoms of benign prostatic hyperplasia (BPH).
* Off-label: Medical expulsive therapy for ureteral calculi.
## Adult Dosing
* **BPH:** 0.4 mg orally once daily. May increase to 0.8 mg daily after 2–4 weeks if inadequate response is observed.
* **Maximum Dose:** 0.8 mg daily.
* **Administration:** Take 30 minutes after the same meal each day. Swallow capsule whole; do not crush or chew.
## Pediatric Dosing
* **Not indicated/Not established.** Clinical safety and efficacy have not been established in the pediatric population.
## Dose Adjustments
* **Renal Impairment:** No dosage adjustment indicated; use with caution in patients with severe renal impairment (CrCl <10 mL/min).
* **Hepatic Impairment:** No dosage adjustment for mild to moderate impairment; use with caution in severe hepatic impairment.
* **CYP3A4 Inhibition:** If used with strong inhibitors (e.g., ketoconazole), clinical caution is required; reduce dose or avoid if necessary.
## Contraindications
* Hypersensitivity to tamsulosin or any component of the formulation.
* History of orthostatic hypotension or syncope.
## Adverse Effects
* **Common:** Dizziness, headache, abnormal ejaculation (retrograde ejaculation), rhinitis, fatigue.
* **Serious:** Orthostatic hypotension (with or without syncope), intraoperative floppy iris syndrome (IFIS) during cataract surgery, priapism (rare), angioedema.
## Key Drug Interactions
* **PDE5 Inhibitors:** Concomitant use (e.g., sildenafil, tadalafil) may result in additive hypotensive effects; initiate at lowest dose.
* **CYP3A4 and CYP2D6 Inhibitors:** Concurrent use with inhibitors (e.g., clarithromycin, paroxetine) may increase tamsulosin plasma concentrations.
* **Antihypertensives:** Potential for additive blood pressure lowering; monitor for symptomatic hypotension.
## Monitoring
* Monitor blood pressure periodically, especially upon initiation or dose escalation.
* Assess symptom improvement (International Prostate Symptom Score - IPSS).
* Screen for prostate cancer prior to initiation of therapy.
## Clinical Pearls
* **Cataract Surgery:** Advise patients to inform their ophthalmologist of current or past tamsulosin use due to the risk of "Intraoperative Floppy Iris Syndrome" (IFIS).
* **First-Dose Syncopy:** Although less common with tamsulosin compared to non-selective alpha-blockers, advise patients to rise slowly from a sitting or lying position.
* **Efficacy:** While 0.4 mg is standard, the 0.8 mg dose provides greater efficacy for some patients but increases the risk of side effects.
