# Urimax (Tamsulosin)
## Overview
Tamsulosin is a selective α1-adrenergic antagonist that relaxes smooth muscle in the bladder neck and prostate. It demonstrates selectivity for the α1A and α1D receptor subtypes, offering bladder-specific effects with less systemic vasodilation compared to non-selective α-blockers.
## Primary Indications
* Signs and symptoms of Benign Prostatic Hyperplasia (BPH).
* Off-label: Medical Expulsive Therapy (MET) for distal ureteral calculi.
## Adult Dosing
* **BPH:** 0.4 mg orally once daily.
* If no response after 2–4 weeks, the dose may be increased to 0.8 mg orally once daily.
* Max dose: 0.8 mg daily.
## Pediatric Dosing
* **Not indicated/established.** While occasionally used off-label for neurogenic bladder in specialized pediatric urology settings under specific protocols, it is not FDA-approved for pediatric use. Dosing is highly variable and requires expert clinical discretion.
## Dose Adjustments
* **Renal Impairment:** No dosage adjustment necessary.
* **Hepatic Impairment:** Use caution in severe impairment; not recommended in patients with Child-Pugh Class C.
## Contraindications
* Hypersensitivity to tamsulosin.
* History of angioedema.
* Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole) in patients who are known CYP2D6 poor metabolizers.
## Adverse Effects
* **Common:** Dizziness, headache, abnormal ejaculation (retrograde), rhinitis, asthenia.
* **Serious:** Orthostatic hypotension (first-dose phenomenon), syncope, Intraoperative Floppy Iris Syndrome (IFIS) during cataract surgery.
## Key Drug Interactions
* **Strong CYP3A4 Inhibitors:** (e.g., ketoconazole, clarithromycin, ritonavir) Increase tamsulosin exposure significantly.
* **PDE5 Inhibitors:** (e.g., sildenafil, tadalafil) Potential for additive blood pressure lowering effects; initiate PDE5 inhibitors at the lowest dose.
* **Antihypertensives:** Potential for additive orthostatic hypotension.
## Monitoring
* Monitor blood pressure periodically, specifically noting symptoms of orthostasis.
* Prior to initiating therapy, rule out prostate cancer.
* Assess for resolution or improvement of LUTS (lower urinary tract symptoms).
## Clinical Pearls
* **Administration:** Take 30 minutes after the same meal each day to ensure consistent bioavailability. Swallow capsules whole; avoid crushing or opening.
* **Surgical Risk:** Advise patients to inform their ophthalmologist of tamsulosin use prior to cataract or glaucoma surgery to prevent IFIS complications.
* **First Dose:** Caution patients regarding potential syncope or dizziness when rising from a sitting or lying position during initial therapy.
* **Onset:** Full clinical benefit in BPH may take up to 2–4 weeks of consistent dosing.
