# Urimax (Tamsulosin)
## Overview
Tamsulosin is a selective α1-adrenoceptor antagonist that preferentially binds to α1A receptors in the prostate, bladder neck, and urethra, facilitating smooth muscle relaxation to improve urinary flow.
## Primary Indications
* Benign Prostatic Hyperplasia (BPH) – symptomatic treatment.
* Off-label: Expulsive therapy for distal ureteral stones.
* Off-label: Retention of urine (in specific acute settings).
## Adult Dosing
* **BPH:** 0.4 mg orally once daily.
* If no response after 2–4 weeks at 0.4 mg, the dose may be increased to 0.8 mg orally once daily.
* **Maximum Dose:** 0.8 mg daily.
## Pediatric Dosing
* Tamsulosin is **not FDA-approved** for pediatric use.
* Use in pediatric neurourologic conditions (e.g., neurogenic bladder) is strictly off-label and requires specialist/urology guidance. Dosing is often weight-based (e.g., 0.05 to 0.1 mg/kg/day), but local institutional protocols must be consulted.
## Dose Adjustments
* **Renal Impairment:** No dosage adjustment indicated; however, use with caution in patients with severe impairment (CrCl < 10 mL/min).
* **Hepatic Impairment:** Not recommended for use in patients with severe hepatic impairment (Child-Pugh Class C).
## Contraindications
* Known hypersensitivity to tamsulosin or any component of the formulation.
* History of angioedema.
## Adverse Effects
* **Common:** Dizziness, headache, abnormal ejaculation (ejaculation failure/retrograde ejaculation), rhinitis, and asthenia.
* **Serious:** Orthostatic hypotension (syncope may occur, especially upon initiation), Intraoperative Floppy Iris Syndrome (IFIS) during cataract surgery.
## Key Drug Interactions
* **Potent CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin):** May significantly increase tamsulosin exposure. Use with caution or avoid.
* **PDE-5 Inhibitors (e.g., sildenafil, tadalafil):** Concurrent use can cause significant symptomatic hypotension. Initiate at the lowest dose of the PDE-5 inhibitor.
* **Other Antihypertensives:** Potential for additive blood pressure-lowering effects.
## Monitoring
* Monitor blood pressure during dose initiation or titration to screen for orthostatic hypotension.
* Baseline and periodic prostate-specific antigen (PSA) levels if monitoring for prostate cancer.
* Digital rectal exam to rule out prostate cancer prior to and during therapy.
## Clinical Pearls
* **Administration:** Administer 30 minutes after the same meal each day to decrease variability in absorption. Swallow capsule whole; do not crush or chew.
* **Cataract Surgery:** Advise patients to inform their ophthalmologist of current or prior tamsulosin use, as it increases the risk of IFIS. If surgery is imminent, some surgeons may recommend temporary discontinuation preoperatively.
* **Efficacy:** While it improves urine flow, it does not typically reduce prostate size.
