# Tranexamic Acid
## Overview
Tranexamic acid is an antifibrinolytic agent that works by reversibly inhibiting the activation of plasminogen to plasmin, thus preventing fibrinolysis.
## Primary Indications
* Hemorrhagic conditions (e.g., menorrhagia, epistaxis, gastrointestinal bleeding)
* Reduction of blood loss during and after surgery
* Treatment of hereditary angioedema
## Adult Dosing
* **Oral:**
* Menorrhagia: 1000-1500 mg TID to QID. Maximum dose typically 4000 mg/day.
* Hereditary angioedema prophylaxis: 1000 mg BID to TID.
* **Intravenous:**
* Surgical procedures: 10-20 mg/kg IV bolus prior to incision, followed by 1-2 mg/kg/hour IV infusion. Dosing may vary significantly based on surgical procedure and local protocol.
* Managing acute bleeding: 500-1000 mg IV TID to QID. Maximum dose typically 4000 mg/day.
## Pediatric Dosing
Dosing in pediatric patients is not well-established and should be based on weight, indication, and clinical judgment, often guided by adult doses and adjusted accordingly. For surgical bleeding, doses may range from 10 mg/kg IV to 25 mg/kg IV.
## Dose Adjustments
* **Renal Impairment:** Dose reduction is necessary.
* Creatinine clearance 50-80 mL/min: Give recommended dose every other day.
* Creatinine clearance 10-50 mL/min: Give recommended dose every day (or 1/2 recommended dose every other day).
* Creatinine clearance <10 mL/min: Give recommended dose every 2 days (or 1/4 recommended dose every day).
## Contraindications
* Active intravascular clotting or thrombosis
* Hypersensitivity to tranexamic acid
* Acquired defective color vision (for oral use; IV use is generally considered safe with caution)
* Subarachnoid hemorrhage (associated with increased risk of cerebral edema and infarction)
## Adverse Effects
* **Common:** Gastrointestinal disturbances (nausea, vomiting, diarrhea), dizziness, headache, menstrual cramps.
* **Serious:** Thrombotic events (deep vein thrombosis, pulmonary embolism, stroke), allergic reactions, visual disturbances, hypotension (with rapid IV infusion).
## Key Drug Interactions
* **Hormonal contraceptives:** Increased risk of thrombosis.
* **Factor VIIa:** Increased risk of thrombosis.
* **Anticoagulants (e.g., warfarin, heparin):** Concurrent use is generally avoided due to increased thrombosis risk. However, in specific situations like concurrent bleeding management, careful monitoring is essential.
## Monitoring
* Thrombotic event risk, especially in patients with risk factors.
* Renal function for dose adjustments.
* Visual changes with long-term oral therapy.
* Bleeding status and efficacy.
## Clinical Pearls
* Tranexamic acid is most effective when initiated early in the course of bleeding.
* IV administration should be by slow infusion to minimize the risk of hypotension and adverse effects.
* Consider tranexamic acid in patients with excessive bleeding that is not life-threatening but causes significant morbidity, such as menorrhagia.
* The risk of thrombosis is a significant concern and must be weighed against the benefit of hemostasis.
