# Tranexamic acid
## Overview
Tranexamic acid is an antifibrinolytic agent that competitively inhibits the activation of plasminogen to plasmin. It is used to control or prevent bleeding in various clinical situations.
## Primary Indications
* **Hemorrhagic conditions:** Prevention and treatment of bleeding associated with hyperfibrinolysis, such as menorrhagia, epistaxis, gastrointestinal bleeding, and hemophilia.
* **Surgical bleeding:** Reduction of blood loss during and after surgery, particularly in specialties like orthopedic, cardiac, and prostate surgeries.
* **Trauma:** Reduction of mortality in patients with severe traumatic bleeding.
## Adult Dosing
Dosing varies significantly based on indication and route of administration.
* **Menorrhagia:**
* Oral: 1 gram (g) three times daily, up to a maximum of 4 g daily, for up to 4 days during menstruation.
* **Gingival bleeding:**
* Oral: 1 g three times daily.
* **Epistaxis:**
* Oral: 1 g three times daily.
* Topical: Solution can be used as a nasal spray or gargle.
* **Hemorrhagic conditions (general/surgical):**
* Intravenous (IV): 1 g to 1.5 g every 6 to 8 hours. Higher doses may be used in specific surgical settings.
* **Major trauma:**
* IV: Loading dose of 1 g over 10 minutes, followed by an infusion of 1 g per hour for up to 8 hours, or until bleeding is controlled. (CRASH-2 trial dosing).

## Pediatric Dosing
Dosing in pediatric patients is less established and often based on adult data or weight-based calculations. Consultation with pediatric specialists is recommended.
* **General guideline (oral):** 20-25 mg/kg/dose every 8 hours, not to exceed adult doses.
* **General guideline (IV):** 10 mg/kg/dose every 6-8 hours.
## Dose Adjustments
* **Renal Impairment:** Dose adjustment is crucial.
* Creatinine clearance (CrCl) 50-80 mL/min: 75% of standard dose.
* CrCl 20-50 mL/min: 50% of standard dose.
* CrCl 10-20 mL/min: 25% of standard dose.
* CrCl <10 mL/min: Consider avoiding or use with extreme caution, potentially 10% of standard dose and prolonged intervals.
* **Hepatic Impairment:** No dose adjustment is typically required.
## Contraindications
* Active intravascular thrombosis or thromboembolic disease (e.g., deep vein thrombosis, pulmonary embolism, arterial thrombosis).
* History of thrombosis.
* Hypersensitivity to tranexamic acid.
* Acquired defective color vision (for long-term oral use).
* Subarachnoid hemorrhage (controversial; risk of vasospasm and cerebral ischemia may outweigh benefits).
## Adverse Effects
* **Common:** Nausea, vomiting, diarrhea, abdominal pain.
* **Serious:**
* Thromboembolic events (stroke, myocardial infarction, deep vein thrombosis, pulmonary embolism).
* Hypersensitivity reactions (anaphylaxis).
* Visual disturbances (especially with long-term oral use).
* Seizures (particularly with rapid IV administration or high doses).
* Hypotension (with rapid IV infusion).
## Key Drug Interactions
* **Hormonal contraceptives (oral, transdermal, vaginal):** Increased risk of thromboembolic events.
* **Factor VIIa (recombinant):** Increased risk of thrombosis.
* **Antifibrinolytics (e.g., aminocaproic acid):** Concurrent use is generally not recommended due to increased thrombotic risk.
* **Estrogens:** May potentiate hypercoagulability.
* **Oral contraceptives:** May potentiate hypercoagulability.
## Monitoring
* **Clinical:** Assess for bleeding and signs of therapeutic response. Monitor for adverse effects, particularly thromboembolic events, visual changes, and GI upset.
* **Laboratory:** Baseline and periodic renal function tests (creatinine) are essential for dose adjustment.
## Clinical Pearls
* Tranexamic acid is most effective when administered early in the bleeding event.
* Rapid IV injection can cause hypotension and increase the risk of seizures. Administer IV doses slowly.
* For oral use, consider dividing the daily dose to improve GI tolerance.
* The risk of thromboembolic events is a significant concern. Carefully weigh the benefits against the risks, especially in patients with pre-existing risk factors for thrombosis.
