# Tranexamic acid
## Overview
Tranexamic acid is an antifibrinolytic agent that competitively inhibits the activation of plasminogen to plasmin, thereby reducing fibrinolysis.
## Primary Indications
* Hemorrhagic disorders.
* Management of menorrhagia.
* Prevention and treatment of bleeding after oral surgery in patients with known coagulation disorders.
* Adjunctive treatment in major gynecological surgery.
* Hemorrhagic complications of heparin therapy.
* Intracranial hemorrhage (particularly subarachnoid hemorrhage).
* Angioedema.
## Adult Dosing
* **Hemorrhagic disorders:** 1 to 1.5 g (1000 to 1500 mg) orally or intravenously every 6 to 8 hours. Total daily dose should not exceed 6 g (6000 mg).
* **Menorrhagia:** 1 g (1000 mg) orally three times daily for up to 4 days during menstruation.
* **Oral surgery (coagulopathy):** 1 g (1000 mg) orally 3 to 4 times daily, starting 2 to 12 hours before surgery and continuing for 2 to 8 days postoperatively. Mouthwash (10 mL of 5% solution TID or QID) may be used adjunctively.
