# Tranexamic Acid
## Overview
Tranexamic acid (TXA) is a synthetic lysine analog that acts as an antifibrinolytic agent. It competitively inhibits the activation of plasminogen to plasmin, thereby preventing the degradation of fibrin clots and stabilizing hemostasis.
## Primary Indications
* **Trauma:** Reduction of mortality in patients with significant hemorrhage (CRASH-2 protocol).
* **Surgery:** Reduction of perioperative blood loss in orthopedic, cardiac, and spinal surgeries.
* **Obstetrics:** Treatment and prevention of postpartum hemorrhage.
* **Menorrhagia:** Treatment of cyclic heavy menstrual bleeding.
* **Epistaxis/Hemoptysis:** Often used topically or systemically for refractory cases.
## Adult Dosing
* **Trauma (CRASH-2):** 1 g IV bolus over 10 minutes, followed by 1 g IV infusion over 8 hours. Administer as soon as possible, ideally within 3 hours of injury.
* **Heavy Menstrual Bleeding:** 1,300 mg orally three times daily for up to 5 days during menses.
* **Surgical Prophylaxis:** Varies widely by specialty; common ranges are 10–30 mg/kg IV bolus followed by variable infusions based on local protocol.
## Pediatric Dosing
* **Trauma:** 15 mg/kg (up to 1 g) IV bolus, followed by 2 mg/kg/hour infusion for up to 8 hours.
* **Cardiac/Surgical:** Dosing is highly variable; common loading doses range from 10–30 mg/kg depending on institutional cardiovascular surgery protocols. Consultation with specialized pediatric pharmacy or anesthesia protocols is required.
## Dose Adjustments
* **Renal Impairment:** Requires dose reduction in patients with CrCl <50 mL/min.
* CrCl 30–50 mL/min: 50% of standard dose or increased dosing interval.
* CrCl <30 mL/min: Use caution; 25% of standard dose or profound interval extension.
* **Hepatic Impairment:** No specific dosage adjustments established, but use with caution.
## Contraindications
* Active intravascular clotting (e.g., DVT, PE, history of thromboembolism) unless the patient is anticoagulated.
* Acquired defective color vision (prevents monitoring of optic nerve toxicity).
* Subarachnoid hemorrhage (risk of cerebral ischemia/vasospasm).
* Hypersensitivity to tranexamic acid.
## Adverse Effects
* **Common:** Nausea, vomiting, diarrhea, abdominal pain.
* **Serious:** Thromboembolic events (PE, DVT, MI, stroke), seizures (particularly with high doses or rapid IV administration), and visual disturbances (e.g., blurred vision, central retinal artery occlusion).
## Key Drug Interactions
* **Procoagulants/Estrogens:** Risk of thrombosis is cumulatively increased when combined with factor IX complex concentrates or estrogen-containing products.
* **Thrombolytics:** Concurrent use with thrombolytic agents (e.g., alteplase) significantly negates the effect of both drugs; avoid combination.
## Monitoring
* **Clinical:** Monitor for signs of new thrombi (leg swelling, chest pain, dyspnea).
* **Neurological:** Monitor for seizures (especially in renal impairment).
* **Ophthalmological:** Baseline and periodic eye exams recommended if long-term therapy is required; stop immediately if visual changes occur.
* **Laboratory:** Renal function tests in patients requiring repeat or ongoing dosing.
## Clinical Pearls
* **Administration:** Rapid IV injection can cause hypotension; always infuse over at least 10 minutes.
* **The "3-Hour Rule":** In major trauma, the efficacy of TXA markedly decreases and potential harm/no benefit may occur if administered later than 3 hours post-injury.
* **Topical Use:** TXA is increasingly used topically (e.g., in mouthwash for dental procedures or soaked gauze for epistaxis) to minimize systemic absorption.
* **Protocol Dependency:** Always defer to institutional clinical guidelines for surgical and ICU dosing, as there is significant variation in "standard" practice protocols.
