# Tranexamic Acid
## Overview
Tranexamic acid (TXA) is a synthetic lysine analog that acts as an antifibrinolytic agent by reversibly blocking lysine binding sites on plasminogen molecules, thereby preventing the conversion of plasminogen to plasmin and inhibiting the breakdown of fibrin clots.
## Primary Indications
* **Trauma-related hemorrhage:** Reduce mortality in adult trauma patients with significant hemorrhage (CRASH-2 protocol).
* **Postpartum hemorrhage (PPH):** Treatment of hemorrhage following childbirth (WOMAN trial).
* **Surgical bleeding:** Prophylaxis or treatment of excessive bleeding in cardiac, orthopedic, and dental procedures.
* **Menorrhagia:** Treatment of heavy menstrual bleeding.
## Adult Dosing
* **Trauma:** 1 g IV loading dose over 10 minutes, followed by a continuous infusion of 1 g over 8 hours (must be administered within 3 hours of injury).
* **Postpartum Hemorrhage:** 1 g IV over 10 minutes; may repeat a second 1 g dose if bleeding continues after 30 minutes.
* **Menorrhagia:** 1,300 mg orally three times daily for up to 5 days during menstruation.
## Pediatric Dosing
* **Trauma/Hemorrhage:** 10–15 mg/kg IV (max 1 g) loading dose, followed by 2–5 mg/kg per hour maintenance infusion. *Note: Pediatric guidelines vary significantly; institutional protocols must be consulted.*
## Dose Adjustments
* **Renal Impairment:** Requires dose reduction in patients with elevated serum creatinine (CrCl < 50 mL/min).
* CrCl 1:15–30 mL/min: 50% of usual dose.
* CrCl < 15 mL/min: 25% of usual dose.
## Contraindications
* Active intravascular clotting (thromboembolic disease).
* Acquired defective color vision (prevents monitoring of retinal toxicity).
* Subarachnoid hemorrhage (theoretical risk of cerebral vasospasm/ischemia).
* Known hypersensitivity to TXA.
## Adverse Effects
* **Common:** Nausea, vomiting, diarrhea, abdominal pain.
* **Serious:** Thromboembolic events (DVT, PE, MI, stroke), seizures (particularly with rapid IV injection or high doses during cardiac surgery), allergic reaction, visual changes (retinal occlusion).
## Key Drug Interactions
* **Estrogens/Hormonal Contraceptives:** Increased risk of thrombosis when used concurrently.
* **Prothrombin Complex Concentrates/Factor IX:** Potential increased risk of thrombosis due to pro-coagulant effects.
## Monitoring
* Monitor for signs of venous or arterial thrombosis (e.g., extremity pain, chest pain, shortness of breath).
* Assess volume status to ensure bleeding is not masked by persistent hypotension if hypovolemia remains unresolved.
* Ophthalmologic exams if long-term treatment is initiated.
* Monitor renal function to guide dose adjustments.
## Clinical Pearls
* **Time-Sensitivity:** Efficacy in trauma and PPH is highly time-dependent; the benefit significantly decreases if administered >3 hours post-injury.
* **Seizure Risk:** Rapid IV injection can trigger seizures. Always administer as a slow infusion over 10 minutes.
* **Compatibility:** TXA is generally compatible with common IV fluids (NS, D5W, LR) but should not be mixed with penicillin or blood products in the same line without verifying compatibility.
