# Tranexamic Acid
## Overview
Tranexamic acid (TXA) is a synthetic lysine analog that acts as an antifibrinolytic agent by competitively inhibiting the activation of plasminogen to plasmin, thereby preventing clot degradation.
## Primary Indications
* **Trauma:** Hemorrhagic shock (CRASH-2 protocol).
* **Surgery:** Reduction of periprocedural blood loss (e.g., cardiac, orthopedic, gynecologic).
* **Obstetrics:** Postpartum hemorrhage.
* **Emergency Medicine:** Epistaxis, menorrhagia, traumatic hyphema.
## Adult Dosing
* **Trauma:** 1 g IV loading dose over 10 minutes, followed by 1 g IV infusion over 8 hours. Administer as soon as possible, ideally within 3 hours of injury.
* **Menorrhagia:** 1.3 g PO TID for up to 5 days during menstruation.
* **Surgical Prophylaxis:** Dosing varies widely by procedure (typically 10–30 mg/kg IV). Consult institutional guidelines.
## Pediatric Dosing
* **Trauma:** 15 mg/kg IV (max 1 g) loading dose, followed by 2 mg/kg/hour infusion for 8 hours (or per trauma protocol).
* **Standard Off-Label Use:** 10–25 mg/kg IV every 6–8 hours depending on indication. *Dosing is highly protocol-dependent; verify with pediatric weight-based guidelines.*
## Dose Adjustments

## Contraindications

## Key Drug Interactions
* **Thrombolytics/Factor IX Complex:** Concurrent use may theoretically antagonize the efficacy of thrombolytic therapy or increase the risk of thrombosis with pro-coagulant agents.
* **Estrogens:** Increased risk of thrombosis if used concomitantly with oral contraceptives.
## Monitoring
* **Clinical:** Monitor for signs of thromboembolism (leg swelling, chest pain, dyspnea) and seizure activity.
* **Laboratory:** Renal function tests in patients with long-term use.
## Clinical Pearls
* **Seizure Risk:** Do not exceed recommended infusion rates; rapid IV injection increases seizure risk, even in patients without a history of epilepsy.
* **Timing:** Efficacy in trauma significantly decreases if administered >3 hours post-injury.
* **Stability:** Once diluted, IV solutions are generally stable for 24 hours at room temperature.
* **Off-Label:** Frequently used topically (e.g., soaked gauze for epistaxis) or orally (off-label compounding) for mucosal bleeding.
