# Tranexamic Acid
## Overview
Tranexamic acid (TXA) is a synthetic lysine analog that acts as a potent antifibrinolytic agent. It competitively inhibits the activation of plasminogen to plasmin, thereby preventing the breakdown of fibrin clots and stabilizing hemostasis.
## Primary Indications
* Treatment or prevention of excessive bleeding in trauma (CRASH-2 protocol).
* Postpartum hemorrhage (WOMAN trial protocol).
* Heavy menstrual bleeding.
* Prophylaxis in surgical procedures (cardiac, orthopedic, dental).
* Hereditary angioedema (off-label).
## Adult Dosing
* **Trauma/Acute Hemorrhage:** 1 g IV loading dose over 10 minutes, followed by 1 g IV infusion over 8 hours (or 1 g IV every 8 hours depending on institutional protocol).
* **Heavy Menstrual Bleeding:** 1.3 g orally three times daily for up to 5 days during menstruation.
* **Surgical Prophylaxis:** Varies widely by specialty; often 10–20 mg/kg IV bolus prior to incision.
## Pediatric Dosing
* **Trauma/Hemorrhage:** 10–15 mg/kg IV bolus (max 1 g), followed by 2–5 mg/kg/hour infusion, or 10 mg/kg IV every 8 hours.
* *Note:* Protocols vary significantly; consultation with pediatric hematology or institutional formularies is mandatory.
## Dose Adjustments
* **Renal Impairment:** Requires dose reduction in patients with CrCl < 50 mL/min.
* CrCl 30–50 mL/min: 10 mg/kg IV daily or 5 mg/kg q12h.
* CrCl < 30 mL/min: 5–10 mg/kg IV every 24–48 hours.
* **Hepatic Impairment:** No formal adjustment required, but use caution.
## Contraindications
* Active intravascular clotting (thromboembolic disease).
* History of deep vein thrombosis (DVT) or pulmonary embolism (PE).
* Acquired defective color vision (prevents monitoring of retinal changes).
* Subarachnoid hemorrhage (risk of cerebral ischemia/seizures).
## Adverse Effects
* **Common:** Nausea, vomiting, diarrhea, abdominal pain.
* **Serious:** Thromboembolic events (MI, DVT, PE, stroke), seizures (particularly with rapid IV injection or high doses), visual disturbances, and allergic skin reactions.
## Key Drug Interactions
* **Hormonal Contraceptives:** Concomitant use with estrogen-containing contraceptives may increase the risk of thrombosis.
* **Thrombolytics/Procoagulants:** Risk of antagonistic effect; avoid combination with Factor IX complex concentrates or anti-inhibitor coagulant complexes due to potential risk of thrombosis.
## Monitoring
* Monitor for signs of thromboembolism (e.g., leg swelling, chest pain, dyspnea).
* Neurological status (seizure risk is higher in cardiac surgery/high-dose settings).
* Renal function (BUN/Cr) to adjust dosing as necessary.
* Ophthalmologic exams if treatment exceeds 7 days (rarely required for acute bleeding).
## Clinical Pearls
* **Seizure Risk:** Rapid IV injection can trigger seizures. Always administer as a slow infusion or slow bolus.
* **Timing:** Efficacy is time-dependent; the "golden hour" for trauma administration is critical. Benefits decrease significantly if administered >3 hours post-injury.
* **Off-label use:** Frequently used for epistaxis or trauma-related hemorrhages, but always check local clinical pathways.
