# Tranexamic Acid
## Overview
Tranexamic acid (TXA) is a synthetic lysine analogue that acts as an antifibrinolytic agent. It competitively inhibits the activation of plasminogen to plasmin, thereby preventing the degradation of fibrin clots.
## Primary Indications
* **Trauma:** Prevention of hemorrhage in adult trauma patients with active or suspected significant bleeding.
* **Surgery:** Reduction of perioperative blood loss in cardiac, orthopedic, and spinal surgeries.
* **Obstetrics:** Treatment of postpartum hemorrhage (PPH) and management of heavy menstrual bleeding.
* **Epistaxis/Dental:** Management of minor bleeding in patients on anticoagulants.
## Adult Dosing
* **Trauma:** 1 g intravenously (IV) over 10 minutes (loading dose), followed by 1 g IV infusion over 8 hours (maintenance). Administer as soon as possible, ideally within 3 hours of injury.
* **Postpartum Hemorrhage:** 1 g IV slowly (1 mL/min). A second 1 g dose may be considered if bleeding continues after 30 minutes.
* **Heavy Menstrual Bleeding:** 1.3 g orally three times daily for up to 5 days during menstruation.
## Pediatric Dosing
* **Trauma:** 15 mg/kg (max 1 g) IV loading dose, followed by a maintenance infusion of 2 mg/kg/hour for 8 hours (or until bleeding stops).

## Dose Adjustments
* **Renal Impairment:** Required for both IV and oral formulations. Accumulation occurs as TXA is primarily renally excreted. Consult current renal dosing charts (e.g., Lexicomp/UpToDate) based on CrCl.

## Contraindications
* History of thromboembolic disease (DVT, PE, cerebral thrombosis).
* Active intravascular clotting.

## Adverse Effects
* **Common:** Nausea, vomiting, diarrhea, abdominal pain.
* **Serious:** Thromboembolic events (PE, DVT, MI, stroke), seizures (particularly with rapid IV injection or high doses), visual disturbances (retinal artery/vein occlusion), and allergic reactions.
## Key Drug Interactions
* **Estrogens/Hormonal Contraceptives:** Increased risk for thrombosis.
* **Thrombolytics:** Reduced effectiveness of thrombolytic therapy (e.g., tPA).
* **Prothrombin Complex Concentrates (PCCs)/Factor IX:** Possible increased risk of thrombotic complications when used concurrently.
## Monitoring
* Monitor for signs/symptoms of thromboembolism (e.g., leg pain, shortness of breath, chest pain, focal neurological deficits).
* Regular ophthalmological exams if long-term therapy is required.
* Monitor renal function in patients with pre-existing impairment.
## Clinical Pearls
* **Rapid Administration:** Avoid rapid IV push; rapid injection can lead to hypotension and increase seizure risk. Always dilute or administer via slow infusion.
* **Timing:** Efficacy in trauma follows the "golden hour" principle; benefit decreases significantly if administered >3 hours post-injury.
* **Topical Use:** TXA is increasingly used topically in surgical fields or via gauze for epistaxis/dental bleeding, though systemic absorption may still occur.
* **Seizure Risk:** High-dose systemic therapy, particularly in patients with intracranial pathology or undergoing cardiac surgery, is associated with a risk of postoperative seizures.
