# Tranexamic Acid
## Overview
Tranexamic acid (TXA) is a synthetic lysine analog that acts as an antifibrinolytic agent by competitively inhibiting the activation of plasminogen to plasmin, thereby preventing fibrin degradation.
## Primary Indications
* Treatment/prevention of hemorrhage due to primary fibrinolysis (e.g., trauma, surgical procedures).
* Heavy menstrual bleeding.
* Hereditary angioedema (prophylaxis).
* Epistaxis or ocular hemorrhage (topical/adjunctive).
## Adult Dosing
* **Trauma (CRASH-2 protocol):** 1 g IV bolus over 10 minutes, followed by 1 g IV infusion over 8 hours. Administer as soon as possible, ideally within 3 hours of injury.
* **Heavy Menstrual Bleeding:** 1300 mg PO TID for up to 5 days during menses.
* **Surgical Hemorrhage:** Dosing varies widely by procedure (typically 10–20 mg/kg IV bolus). Refer to institution-specific protocols.
## Pediatric Dosing
* **Trauma/Systemic Hemorrhage:** 10–15 mg/kg IV bolus (max 1 g), followed by 2–10 mg/kg/hr infusion (off-label).
* **Hereditary Angioedema:** 10 mg/kg PO TID (max 1.5 g/dose).

## Dose Adjustments

## Contraindications
* Hypersensitivity to the drug.

* History of intracranial bleeding.
* Acquired color vision disturbances (prevents monitoring of retinal toxicity).
* Subarachnoid hemorrhage (concurrent risk of cerebral vasospasm).
## Adverse Effects
* **Common:** Nausea, vomiting, diarrhea, abdominal pain, headache, back pain.
* **Serious:** Thromboembolic events (DVT, PE, MI, stroke), seizures (especially with rapid IV injection or high doses), visual disturbances (retinal issues), allergic reactions (anaphylaxis).
## Key Drug Interactions
* **Thrombolytics/Fibrinolytics:** Antagonistic effect; avoid concurrent use.
* **Estrogens/Hormonal Contraceptives:** Potential increased risk of thrombosis.
* **Factor IX complex/Prothrombin Complex Concentrates:** Theoretical increased risk of thrombosis.
## Monitoring
* **Clinical:** Monitor for signs of persistent bleeding or new thromboembolic events (DVT/PE).
* **Ophthalmologic:** Consider baseline and periodic eye exams if used long-term for hereditary angioedema.
* **Renal:** Monitor CrCl in patients with renal impairment.
## Clinical Pearls
* **Avoid Rapid IV Push:** Rapid administration can cause hypotension and seizures. Infuse slowly.
* **Timing:** Efficacy in trauma is highly time-dependent; clinical benefit decreases significantly after 3 hours post-injury.
* **Off-label Uses:** Commonly used off-label topically in orthopedic or plastic surgeries to reduce localized bleeding; check local protocols for concentrations.
