# Tobramycin eye drop
## Overview
- **Classification**: Aminoglycoside antibiotic
- **Mechanism**: Inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit. Bactericidal against susceptible organisms.
## Primary Indications
1. **Bacterial Conjunctivitis**: Treatment of superficial bacterial infections of the eye.
2. **Bacterial Blepharitis**: Management of bacterial infections affecting the eyelids.
3. **Bacterial Keratitis/Corneal Ulcers**: Treatment of severe bacterial corneal infections.
## Adult Dosing
### Standard Dosing
**Bacterial Conjunctivitis/Blepharitis**
- **Dose**: **1-2 drops** (0.3% ophthalmic solution)
- **Frequency**: Every **4-6 hours**.
- **Route**: Ophthalmic
- **Duration**: Typically **7-10 days**, or as directed by physician.
**Severe Ocular Infection (e.g., Bacterial Keratitis)**
- **Dose**: **2 drops** (0.3% ophthalmic solution)
- **Frequency**: Initially, every **15 minutes** for the first hour, then every **30 minutes** for **6-8 hours**, then every **hour** for up to **24 hours**. Reduce frequency to every **4 hours** as infection improves.

### Dose Adjustments
- **Renal Impairment**: Not systemically absorbed significantly; no adjustment needed.

## Pediatric Dosing
### Neonates (0-28 days)
- **Indication**: Superficial bacterial ocular infections.
- **Dose**: **1 drop** (0.3% solution)
- **Frequency**: Every **4-6 hours** initially, or as clinically indicated.
- **Maximum**: Use shortest effective duration possible.
- **Special Notes**: Use with caution. Monitor for local irritation or systemic effects (extremely rare).
### Infants (1-12 months)
- **Indication**: Superficial bacterial ocular infections.
- **Dose**: **1 drop** (0.3% solution)
- **Frequency**: Every **4-6 hours**.
- **Maximum**: No more than **6 times daily** per eye.
### Children (1-12 years)
- **Indication**: Superficial bacterial ocular infections.
- **Dose**: **1 drop** (0.3% solution) every **4-6 hours**. For severe infections, follow adult severe infection protocol.
- **Maximum**: Equivalent to adult dosing for severe infections.
### Adolescents (13-18 years)
- **Indication**: Superficial bacterial ocular infections.
- **Dose**: Follow adult dosing recommendations.
- **Maximum**: Same as adult maximum doses.
## Safety Information
### Contraindications
- **Absolute**: Hypersensitivity to tobramycin or other aminoglycosides.
- **Absolute**: Hypersensitivity to any component of the formulation.
### Common Adverse Effects
- **Very Common (>10%)**: Ocular discomfort, stinging, burning upon instillation.
- **Common (1-10%)**: Eyelid itching, eyelid swelling, conjunctival erythema.
- **Serious but Rare**: Localized ocular toxicity (e.g., punctate keratitis), systemic aminoglycoside toxicity (e.g., ototoxicity, nephrotoxicity) – extremely rare with ophthalmic use.
### Key Drug Interactions
- **Other topical ophthalmic medications**: Administer at least **5-10 minutes apart** to avoid dilution or washout.
- **Contact Lenses**: Avoid use during treatment. If necessary, remove before drops and wait **15 minutes** before reinsertion.
## Monitoring & Follow-up
- **Before Treatment**: Assess signs/symptoms of bacterial infection.
- **During Treatment**: Monitor for clinical improvement of infection within **2-3 days**.
- **Clinical Signs**: Watch for worsening pain, redness, vision changes, or signs of allergic reaction.
## Clinical Pearls
- 💡 **Proper Technique**: Wash hands. Tilt head back, pull lower eyelid down, instill drop, close eye gently.
- 💡 **Dropper Tip**: Do not touch the dropper tip to the eye, fingers, or any surface to prevent contamination.
- 💡 **Compliance**: Advise patients to complete the full course of therapy, even if symptoms improve, to prevent recurrence.
- 💡 **Storage**: Store at room temperature, away from light.
