# Tobramycin Eye Drop
## Overview
Tobramycin is an aminoglycoside antibiotic indicated for the topical treatment of external ocular and adnexal infections caused by susceptible bacteria. It is available as a 0.3% ophthalmic solution or ointment.
## Primary Indications
Treatment of superficial ocular infections involving the conjunctiva and/or cornea caused by susceptible strains of *Staphylococcus aureus*, *Staphylococcus epidermidis*, *Streptococcus* species, *Pseudomonas aeruginosa*, *Escherichia coli*, *Klebsiella pneumoniae*, *Enterobacter aerogenes*, *Proteus mirabilis*, and *Haemophilus influenzae*.
## Adult Dosing
* **Mild to Moderate Infections:** Instill 1 to 2 drops into the affected eye(s) every 4 hours.
* **Severe Infections:** Instill 2 drops into the affected eye(s) hourly initially; reduce frequency as infection is controlled.
* **Ointment:** Apply a 0.5-inch ribbon into the affected eye(s) 2 to 3 times daily, or utilize in conjunction with the solution at bedtime.
## Pediatric Dosing
* **Neonates/Infants/Children:** Safety and efficacy have been established in patients 2 months and older. Dosage is the same as adults (1–2 drops every 4 hours).
* *Note:* Use in neonates younger than 2 months is off-label; clinical judgment is required.
## Dose Adjustments
No systemic renal or hepatic dose adjustments are necessary for topical ophthalmic administration.
## Contraindications
Hypersensitivity to tobramycin or any component of the formulation.
## Adverse Effects
* **Common:** Localized ocular toxicity and hypersensitivity (e.g., lid itching, lid swelling, conjunctival erythema).
* **Rare:** Punctate keratitis, lacrimation, or hypersensitivity reactions, including ocular anaphylaxis.
## Key Drug Interactions
Concomitant use of other topical ophthalmic products is generally safe, but if using multiple drops, space administrations by at least 5 to 10 minutes to prevent washout. Avoid systemic aminoglycoside therapy concurrently to minimize potential for cumulative toxicity, though systemic absorption is negligible with topical application.
## Monitoring
* **Clinical Efficacy:** Monitor resolution of symptoms (discharge, redness, irritation).
* **Duration:** Prolonged use may lead to overgrowth of non-susceptible organisms, including fungi. If no improvement is seen after 7 days, reassess the diagnosis/pathogen sensitivity.
## Clinical Pearls
* **Administration:** Instruct patients to avoid touching the dropper tip to the eye, eyelid, or any surface to prevent contamination of the solution.
* **Contact Lenses:** Patients should not wear contact lenses during treatment of an active ocular infection.
* **Storage:** Store at 15°C to 25°C (59°F to 77°F).
* **Resistance:** Excessive use contributes to the development of bacterial resistance; reserve for documented or highly suspected bacterial etiology.
