# Tobramycin Eye Drops
## Overview
Tobramycin is an aminoglycoside antibiotic indicated for topical treatment of external ocular infections. It acts by inhibiting bacterial protein synthesis. It is bacteriocidal and effective against *Pseudomonas aeruginosa* and *Staphylococcus* species.
## Primary Indications
* Bacterial conjunctivitis.
* Blepharitis (off-label).
* Keratitis (secondary to susceptible organisms).
## Adult Dosing
* **Mild to Moderate Infection:** 1–2 drops in the affected eye(s) every 4 hours.
* **Severe Infection:** 2 drops in the affected eye(s) every hour initially, tapering dose as improvement occurs.
* **Duration:** Typically 7 days. Do not exceed beyond the resolution of infection due to risk of resistance or fungal overgrowth.
## Pediatric Dosing
* **Age ≥ 2 months:** Same as adult dosing (1–2 drops every 4 hours).
* **Safety Note:** Efficacy and safety in neonates (< 2 months) have not been established. Use with caution.
## Dose Adjustments
* No systemic renal or hepatic dose adjustments are required for topical ophthalmic administration due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to any component of the medication or other aminoglycosides.
## Adverse Effects
* **Common:** Localized ocular irritation, burning, stinging, itching, or erythema.
* **Serious:** Hypersensitivity reactions, corneal ulceration (rare), and potential for development of resistant organisms/superinfection with prolonged use.
## Key Drug Interactions
* Minimal systemic absorption makes clinically significant drug-drug interactions unlikely.
* Advise patients to wait at least 5–10 minutes between the application of different ophthalmic medications.
## Monitoring
* Monitor for clinical resolution of symptoms.
* Discontinue and re-evaluate if condition worsens or if signs of sensitization (itching, lid swelling) occur.
## Clinical Pearls
* **Administration:** Instruct patient to wash hands; avoid touching the tip of the dropper to the eye or any surface to prevent contamination.
* **Contact Lenses:** Patients should not wear contact lenses during treatment for ocular surface infections.
* **Formulation:** Available as a solution (0.3%). Ensure clear distinction from Tobramycin/Dexamethasone combination products; do not use steroids if a viral or fungal etiology is suspected.
* **Resistance:** Tobramycin has good activity against *Pseudomonas* but is less effective against some *Streptococcus* species compared to other antibiotics.
