# Tobramycin Eye Drop
## Overview
Tobramycin is an aminoglycoside antibiotic indicated for the treatment of external ocular infections caused by susceptible bacteria. It acts by inhibiting bacterial protein synthesis. It is available as a 0.3% ophthalmic solution.
## Primary Indications
Treatment of superficial ocular infections involving the conjunctiva and/or cornea caused by susceptible strains of microorganisms (e.g., *Staphylococcus aureus*, *Staphylococcus epidermidis*, *Streptococcus* species, *Pseudomonas aeruginosa*).
## Adult Dosing
* **Mild to Moderate Infection:** Instill 1–2 drops into the affected eye(s) every 4 hours.
* **Severe Infection:** Instill 2 drops into the affected eye(s) hourly; reduce frequency as infection is controlled.
* *Note:* Local institutional protocols may vary regarding frequency; follow internal guidelines for clinical severity.
## Pediatric Dosing
Neonates (≥ 2 months) and children: Same as adult dosing. Safety and efficacy in infants < 2 months old have not been established.
## Dose Adjustments
No specific dosage adjustments are required for renal or hepatic impairment due to minimal systemic absorption with ophthalmic administration.
## Contraindications
Hypersensitivity to tobramycin or any component of the formulation.
## Adverse Effects
* **Common:** Localized ocular toxicity, including lid itching, lid swelling, and conjunctival erythema.
* **Serious:** Potential for hypersensitivity reactions (e.g., anaphylaxis, Stevens-Johnson syndrome) and secondary fungal infections with prolonged use.
## Key Drug Interactions
Concurrent use of other topical ophthalmic products is generally acceptable. If using multiple ophthalmic medications, wait at least 5–10 minutes between administrations to prevent washout of the prior dose.
## Monitoring
* Monitor for clinical resolution of infection (e.g., decreased discharge, reduced redness).
* Assess for signs of local irritation or hypersensitivity.
* Discontinue use if symptoms persist beyond 7–10 days, as prolonged use may lead to overgrowth of non-susceptible organisms, including fungi.
## Clinical Pearls
* **Administration:** Instruct patient to avoid contact of the dropper tip with the eye, eyelid, or any other surface to prevent contamination of the solution.
* **Contact Lenses:** Do not wear contact lenses during treatment due to the underlying infection; soft contact lenses may absorb the preservative (benzalkonium chloride).
* **Efficacy:** Tobramycin has good activity against *Pseudomonas aeruginosa*, often making it a preferred choice when this pathogen is suspected.
