# Tobramycin Ophthalmic Solution/Ointment
## Overview
Tobramycin is an aminoglycoside antibiotic used topically for ocular infections. It inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit. It provides broad-spectrum coverage against many Gram-negative organisms (specifically *Pseudomonas aeruginosa*) and some Gram-positive bacteria (e.g., *Staphylococcus aureus*).
## Primary Indications
* Bacterial conjunctivitis
* Blepharitis
* Dacryocystitis
## Adult Dosing
* **Solution (0.3%):** Instill 1 to 2 drops into the affected eye(s) every 4 hours. In severe infections, instill 2 drops every hour until improvement, then taper frequency.
* **Ointment (0.3%):** Apply approximately a 1/2-inch ribbon into the affected eye(s) 2 to 3 times daily. In severe infections, apply every 3 to 4 hours.
## Pediatric Dosing
* **Neonates, Infants, Children, and Adolescents:** Same as adult dosing. Safety and efficacy are established for patients ≥2 months of age.
## Dose Adjustments
* **Renal/Hepatic Impairment:** No dose adjustments required for topical ophthalmic administration due to negligible systemic absorption.
## Contraindications
* Hypersensitivity to any component of the product (tobramycin or other aminoglycosides).
## Adverse Effects
* **Common:** Localized ocular irritation, burning, stinging, itching, or inflammation.
* **Serious (Rare):** Hypersensitivity reactions, eyelid redness/edema, corneal epithelial erosion, or corneal ulceration with prolonged use.
## Key Drug Interactions
* Clinically significant systemic drug interactions are unlikely due to the low systemic absorption of ophthalmic tobramycin.
* Avoid concomitant use of other topical ophthalmic aminoglycosides due to the risk of additive toxicity.
## Monitoring
* Monitor for signs of treatment failure (worsening redness, pain, or discharge) after 48–72 hours.
* Monitor for signs of hypersensitivity (e.g., severe itching, swelling of the conjunctiva).
* Prolonged use may result in the overgrowth of non-susceptible organisms, including fungi. Limit therapy duration to the minimum necessary.
## Clinical Pearls
* **Proper Administration:** Instruct the patient to wash hands before use and avoid touching the tip of the dropper to the eye, eyelids, or any surface to prevent contamination.
* **Contact Lenses:** Patients should generally avoid wearing contact lenses while experiencing signs of bacterial conjunctivitis or while using the medication.
* **Combination Products:** Be aware that tobramycin is often formulated with corticosteroids (e.g., TobraDex). Ensure the correct product is selected based on the clinical need for anti-inflammatory therapy vs. antibiotic monotherapy.
* **Stability:** Store at room temperature (15°C to 25°C).
