# Tradjenta (linagliptin)
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor used to improve glycemic control in adults with type 2 diabetes mellitus. It works by increasing incretin levels, which in turn increases insulin release and decreases glucagon secretion in a glucose-dependent manner.
## Primary Indications
Adjunctive therapy to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
## Adult Dosing
The recommended dose is 5 mg orally once daily.
## Pediatric Dosing
Dosing has not been established in pediatric patients.
## Dose Adjustments
No dose adjustment is required for patients with renal or hepatic impairment.
## Contraindications
* Known hypersensitivity to linagliptin or any excipients.
* History of serious hypersensitivity reactions to DPP-4 inhibitors.
## Adverse Effects
The most common adverse effects include nasopharyngitis, diarrhea, and upper respiratory tract infection. More serious adverse effects include:
* Hypoglycemia (especially when used in combination with sulfonylureas or insulin)
* Hypersensitivity reactions
* Pancreatitis
* Joint pain (arthralgia)
## Key Drug Interactions
* **CYP3A4 Inducers (e.g., rifampin, carbamazepine, phenytoin, St. John's wort):** May decrease linagliptin exposure and efficacy. Coadministration is not recommended.
* **Sulfonylureas and Insulin:** Increased risk of hypoglycemia. Consider a lower dose of the sulfonylurea or insulin.
## Monitoring
* Blood glucose levels to assess efficacy and guide dosing adjustments of other antidiabetic agents.
* Monitor for signs and symptoms of pancreatitis (persistent severe abdominal pain, with or without vomiting).
* Monitor for signs and symptoms of hypersensitivity reactions.
## Clinical Pearls
* Linagliptin does not require dose adjustment in renal or hepatic impairment, making it a convenient option for patients with these comorbidities.
* It can be taken with or without food.
* Due to the risk of severe and persistent arthralgia, consider discontinuing if severe joint pain develops.
