# Tradename: Tradjenta
## Overview
Tradjenta (linagliptin) is a dipeptidyl peptidase-4 (DPP-4) inhibitor used to improve glycemic control in adults with type 2 diabetes mellitus. It works by increasing incretin levels, which are hormones that stimulate insulin secretion and decrease glucagon production in a glucose-dependent manner.
## Primary Indications
* Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
* Can be used as monotherapy or in combination with other antidiabetic agents such as metformin, sulfonylureas, thiazolidinediones, or insulin.
## Adult Dosing
* The usual dose is 5 mg orally once daily.
* Tradjenta can be taken with or without food.
## Pediatric Dosing
* The safety and efficacy of Tradjenta in pediatric patients have not been established.
## Dose Adjustments
* No dose adjustment is required in patients with mild, moderate, or severe renal impairment.
* No dose adjustment is required in patients with end-stage renal disease requiring dialysis.
* No dose adjustment is required in patients with hepatic impairment.
## Contraindications
* Known hypersensitivity to linagliptin or any excipient in the formulation.
## Adverse Effects
* **Common:** Nasopharyngitis, diarrhea, cough.
* **Serious:** Hypersensitivity reactions (anaphylaxis, angioedema, urticaria, rash), severe and disabling arthralgia (joint pain), bullous pemphigoid, pancreatitis.
* **Hypoglycemia:** Risk is low when used as monotherapy but may increase when used in combination with sulfonylureas or insulin.
## Key Drug Interactions
* **CYP3A4 Inducers:** Strong inducers of CYP3A4 (e.g., rifampin, carbamazepine, phenytoin, St. John's wort) can decrease linagliptin exposure and efficacy. Coadministration is generally not recommended.
* **Metformin and Sulfonylureas:** Concomitant use may increase the risk of hypoglycemia. Dose adjustment of metformin or sulfonylureas may be necessary.
## Monitoring
* **Glycemic Control:** Monitor HbA1c periodically to assess therapeutic effect.
* **Renal Function:** Although no dose adjustment is needed, monitor renal function as clinically indicated, especially in patients with pre-existing renal impairment.
* **Signs of Hypersensitivity/Pancreatitis:** Educate patients on the signs and symptoms of hypersensitivity reactions and pancreatitis and advise them to seek immediate medical attention if these occur.
## Clinical Pearls
* Tradjenta is eliminated primarily via the biliary and intestinal routes and does not require dose adjustment in renal impairment, making it a convenient option for patients with varying degrees of kidney function.
* Patients should be counseled on signs and symptoms of hypersensitivity reactions and pancreatitis.
* Due to the risk of severe and disabling joint pain, consider discontinuing if severe joint pain develops.
