# Tradename linagliptin
## Overview
Linagliptin is an oral dipeptidyl peptidase-4 (DPP-4) inhibitor used to improve glycemic control in patients with type 2 diabetes mellitus. It works by increasing incretin levels, which leads to increased insulin release and decreased glucagon secretion in a glucose-dependent manner.
## Primary Indications
* Type 2 diabetes mellitus, as an adjunct to diet and exercise.
* Can be used as monotherapy or in combination with other oral antidiabetic agents (e.g., metformin, sulfonylureas, thiazolidinediones) or insulin.
## Adult Dosing
The recommended dose is **5 mg once daily**. Linagliptin can be taken with or without food.
## Pediatric Dosing

## Dose Adjustments
No dose adjustment is necessary for patients with **renal impairment** (mild, moderate, or severe) or **end-stage renal disease (ESRD)** requiring dialysis.

## Contraindications

## Adverse Effects

* **Hypersensitivity reactions:** Including urticaria, rash, angioedema, and potentially severe dermatologic reactions.
* **Pancreatitis:** Cases of acute pancreatitis have been reported.

* **Nasopharyngitis:** Upper respiratory tract infection symptoms.
## Key Drug Interactions
* **CYP3A4 Inducers:** Strong inducers of CYP3A4 (e.g., rifampin, carbamazepine, phenytoin, phenobarbital) may decrease linagliptin exposure and efficacy. If coadministration is necessary, monitor glycemic control closely.
* **Sulfonylureas and Insulin:** Increased risk of hypoglycemia. Dose reduction of the sulfonylurea or insulin may be necessary.
## Monitoring
* **HbA1c:** Regularly monitor to assess glycemic control.
* **Renal function:** While no dose adjustment is needed, baseline and periodic monitoring may be prudent in the context of overall diabetes management.
* **Signs and symptoms of hypersensitivity reactions and pancreatitis.**
## Clinical Pearls
* Linagliptin is unique among DPP-4 inhibitors as it is primarily eliminated unchanged via the bile and intestine, thus requiring no dose adjustment in renal impairment.
* Consider patient's risk for hypoglycemia when choosing combination therapy.
