# Tradjenta (linagliptin)
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor used to improve glycemic control in adults with type 2 diabetes mellitus. It increases incretin levels, which leads to increased insulin release and decreased glucagon levels in a glucose-dependent manner.
## Primary Indications
Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
## Adult Dosing
The recommended dose of linagliptin is 5 mg orally once daily.
## Pediatric Dosing
Dosing for pediatric patients has not been established.
## Dose Adjustments
No dose adjustment is required for patients with renal or hepatic impairment.
## Contraindications
Linagliptin is contraindicated in patients with a history of serious hypersensitivity reaction to linagliptin or any of its excipients.
## Adverse Effects
Common adverse effects include nasopharyngitis and diarrhea. Although rare, serious hypersensitivity reactions, angioedema, and urticaria can occur. Pancreatitis is a potential serious adverse effect.
## Key Drug Interactions
* **CYP3A4 Inducers:** Strong inducers of CYP3A4 (e.g., rifampin, carbamazepine, phenytoin, St. John's wort) may decrease linagliptin exposure and efficacy. Doses of 10 mg daily may be considered if co-administered with strong CYP3A4 inducers, although efficacy is uncertain.
