# Tradjenta (linagliptin)
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor used to improve glycemic control in patients with type 2 diabetes mellitus. It works by increasing the levels of incretin hormones, which stimulate insulin release and suppress glucagon secretion in a glucose-dependent manner.
## Primary Indications
Type 2 diabetes mellitus, as an adjunct to diet and exercise, to improve glycemic control.
## Adult Dosing
The recommended dose of linagliptin is 5 mg orally once daily.
## Pediatric Dosing
Safety and efficacy have not been established in pediatric patients.
## Dose Adjustments
No dose adjustment is necessary for patients with renal impairment, including those on dialysis. No dose adjustment is necessary for patients with hepatic impairment.
## Contraindications
Known hypersensitivity to linagliptin or any excipients in the formulation.
## Adverse Effects
* **Common:** Nasopharyngitis, diarrhea.
* **Serious:** Hypoglycemia (especially when used with sulfonylureas or insulin), pancreatitis, severe skin reactions (e.g., bullous pemphigoid), hypersensitivity reactions, joint pain.
## Key Drug Interactions
* **Sulfonylureas & Insulin:** Increased risk of hypoglycemia; consider a lower dose of sulfonylurea or insulin.
* **CYP3A4 Inducers (e.g., rifampin):** May decrease linagliptin exposure and efficacy. Monitor glycemic control closely.
## Monitoring
* HbA1c: Regularly monitor glycemic control.
* Renal and hepatic function: Baseline and periodically as clinically indicated.
* Signs and symptoms of pancreatitis and hypersensitivity reactions.
## Clinical Pearls
* Linagliptin is primarily eliminated unchanged in the bile and gut, leading to no required dose adjustment in renal impairment, which is a key advantage over other DPP-4 inhibitors.
* It can be taken with or without food.
* Due to the risk of hypoglycemia, patients on concomitant sulfonylureas or insulin should be educated on the signs and symptoms and advised to report them.
