# Tradjenta (linagliptin)
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor used to improve glycemic control in adults with type 2 diabetes mellitus.
## Primary Indications
Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
## Adult Dosing
The recommended dose of linagliptin is 5 mg orally once daily. This dose can be taken with or without food.
## Pediatric Dosing
Linagliptin is not approved for use in pediatric patients.
## Dose Adjustments
No dose adjustment is required for patients with renal impairment or end-stage renal disease (ESRD).
## Contraindications
* Hypersensitivity to linagliptin or any excipient in the formulation.
* History of angioedema with DPP-4 inhibitors.
## Adverse Effects
Common adverse effects include nasopharyngitis and upper respiratory tract infections. More serious potential adverse effects include:
* Hypoglycemia (especially when used in combination with sulfonylureas or insulin)
* Hypersensitivity reactions (including anaphylaxis, angioedema, and urticaria)
* Severe and disabling joint pain
* Bullous pemphigoid
## Key Drug Interactions
* **CYP3A4 Inducers (e.g., rifampin):** May decrease linagliptin exposure and efficacy. Concomitant use is not recommended.
* **Sulfonylureas and Insulin:** Increased risk of hypoglycemia; consider a lower dose of the sulfonylurea or insulin.
## Monitoring
* Monitor for signs and symptoms of hypersensitivity reactions.
* Monitor blood glucose for glycemic control and assess for hypoglycemia, particularly when used with other glucose-lowering agents.
* Monitor for severe joint pain.
## Clinical Pearls
* Linagliptin does not require dose adjustment for renal impairment, making it a convenient option for patients with varying degrees of kidney function.
* It is generally associated with a low risk of hypoglycemia when used as monotherapy.
* Consider the risk of pancreatitis and angioedema associated with DPP-4 inhibitors.
