# Tradjenta (linagliptin)
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor used to improve glycemic control in adults with type 2 diabetes mellitus. It works by increasing incretin levels, which leads to increased insulin release and decreased glucagon secretion in a glucose-dependent manner.
## Primary Indications
* Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
## Adult Dosing
* **Recommended dose:** 5 mg orally once daily.
* Can be taken with or without food.
## Pediatric Dosing
* Safety and efficacy in pediatric patients have not been established.
## Dose Adjustments
* No dose adjustment is necessary for patients with renal impairment.
* No dose adjustment is necessary for patients with hepatic impairment.
## Contraindications
* History of serious hypersensitivity reaction to linagliptin or any excipient in the formulation.
## Adverse Effects
* **Common:** Nasopharyngitis, upper respiratory tract infection.
* **Serious:** Hypersensitivity reactions (anaphylaxis, angioedema, urticaria, rash), severe and disabling arthralgia, bullous pemphigoid.
* **Events associated with DPP-4 inhibitors:** Pancreatitis, hypoglycemia (especially when used with sulfonylureas or insulin).
## Key Drug Interactions
* **CYP3A4 Inducers (e.g., rifampicin, carbamazepine, phenytoin, phenobarbital):** May decrease linagliptin exposure and efficacy. Coadministration is not recommended.
* **Sulfonylureas and Insulin:** Increased risk of hypoglycemia; consider dose reduction of sulfonylurea or insulin.
## Monitoring
* **Glycemic control:** Monitor HbA1c levels regularly.
* **Renal function:** Not typically required for dose adjustment, but general monitoring is advised.
* **Signs of hypersensitivity reactions:** Monitor for signs and symptoms of serious hypersensitivity.
* **Signs of pancreatitis:** Monitor for severe, persistent abdominal pain.
## Clinical Pearls
* Linagliptin is primarily eliminated unchanged by the liver and does not require dose adjustment in renal impairment, making it a convenient option for patients with chronic kidney disease.
* Patients should be educated about the signs and symptoms of hypersensitivity reactions and instructed to discontinue linagliptin and seek medical attention immediately if they occur.
* Educate patients about the risk of hypoglycemia when linagliptin is used in combination with sulfonylureas or insulin and advise them to monitor blood glucose levels closely.
