# **Varenicline** (Chantix)
## Overview
Varenicline is a partial nicotine receptor agonist that works by binding to nicotinic acetylcholine receptors in the brain. This helps reduce cravings and withdrawal symptoms associated with smoking cessation and also blocks the rewarding effects of nicotine if smoking resumes.
## Primary Indications
Smoking cessation.
## Adult Dosing
**Initiation (Weeks 1-2):**
* Days 1-3: 0.5 mg once daily
* Days 4-7: 0.5 mg twice daily
* Day 8 through the end of Week 2: 1 mg twice daily
**Continuation (Weeks 3-12):**
* 1 mg twice daily
**Maximum Dose:** 1 mg twice daily.
## Pediatric Dosing
Varenicline is generally not recommended for use in pediatric patients. Safety and efficacy have not been established.
## Dose Adjustments
* **Renal Impairment:**
* **Mild to Moderate (CrCl ≥ 30 mL/min):** No dose adjustment required.
* **Severe (CrCl < 30 mL/min):** Recommended dose is 1 mg once daily.
* **Hepatic Impairment:** No dose adjustment required.
## Contraindications
* Hypersensitivity to varenicline or any component of the formulation.
* History of suicidal ideation or behavior.
## Adverse Effects
Common adverse effects include nausea, insomnia, abnormal dreams, headache, and decreased appetite. Serious neuropsychiatric events such as suicidal ideation, suicide attempts, and completed suicides have been reported.
## Key Drug Interactions
* **Nicotine Replacement Therapy (NRT):** Concomitant use with NRT may increase the risk of adverse events, including cardiovascular events. Monitor closely.
* **CYP1A2 Inhibitors:** Varenicline is a weak inhibitor of CYP1A2. Use with caution and consider dose reduction if co-administered with strong CYP1A2 inhibitors.
## Monitoring
* Monitor for mood changes, suicidal ideation, and behavioral changes.
* Assess efficacy in smoking cessation.
* Monitor renal function in patients with severe renal impairment.
## Clinical Pearls
* Smoking cessation should ideally begin 1-2 weeks after starting varenicline, on a target quit date.
* The full 12-week treatment course is recommended, with an additional 12 weeks of treatment considered for those who have successfully quit.
* Patients should be advised to report any new or worsening psychiatric symptoms or suicidal thoughts immediately. Nausea is common, and the titration schedule helps to mitigate this.
