# Linagliptin
## Overview
Linagliptin is an oral dipeptidyl peptidase-4 (DPP-4) inhibitor indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
## Primary Indications
Type 2 Diabetes Mellitus.
## Adult Dosing
The usual dose is 5 mg orally once daily.
## Pediatric Dosing
Linagliptin is not recommended for use in pediatric patients.
## Dose Adjustments
No dose adjustment is required for patients with renal impairment or hepatic impairment.
## Contraindications
* Known hypersensitivity to linagliptin.
* History of angioedema related to DPP-4 inhibitor therapy.
## Adverse Effects
The most common adverse effects include nasopharyngitis and upper respiratory tract infection.
Serious adverse effects include:
* Pancreatitis: Patients should be advised of the characteristic persistent abdominal pain, which may be severe and radiate to the back, with or without vomiting.
* Hypoglycemia: Risk of hypoglycemia is increased when used in combination with a sulfonylurea or insulin.
* Hypersensitivity reactions: Including rash, urticaria, and angioedema.
* Severe and disabling arthralgia.
## Key Drug Interactions
* **CYP3A4 Inhibitors/Inducers:** Linagliptin is minimally metabolized by CYP3A4. Strong inducers of CYP3A4 (e.g., rifampin) may reduce linagliptin efficacy; use with caution.
* **Sulfonylureas and Insulin:** Concomitant use may increase the risk of hypoglycemia. Dose reduction of the sulfonylurea or insulin may be necessary.
## Monitoring
* HbA1c: To assess glycemic control.
* Renal function: Although no dose adjustment is needed, monitoring is generally recommended for diabetic patients.
* Pancreatic enzyme levels: If pancreatitis is suspected.
## Clinical Pearls
* Linagliptin is unique among DPP-4 inhibitors as it is primarily eliminated unchanged by the bile and gut, thus not requiring dose adjustment in patients with renal impairment.
* It can be taken with or without food.
* Consider the risks of pancreatitis and hypersensitivity reactions.
