# Tab-genixim
## Overview
Tab-genixim is a brand name for a hypothetical third-generation cephalosporin antibiotic. It is intended for the treatment of bacterial infections.
## Primary Indications
* **Community-Acquired Pneumonia (CAP):** Caused by susceptible organisms.
* **Acute Bacterial Exacerbations of Chronic Bronchitis (ABECB):** Caused by susceptible organisms.
* **Acute Maxillary Sinusitis:** Caused by susceptible organisms.
## Adult Dosing
* **CAP & ABECB:** 500 mg orally every 12 hours for 5-7 days.
* **Acute Maxillary Sinusitis:** 500 mg orally every 12 hours for 10 days.
Maximum daily dose: 1000 mg.
## Pediatric Dosing
Dosing for pediatric patients is not established for this hypothetical drug. Use with caution and consult specialized pediatric infectious disease resources.
## Dose Adjustments
* **Renal Impairment:**
* CrCl 30-49 mL/min: 500 mg orally every 24 hours.
* CrCl < 30 mL/min: 500 mg orally every 48 hours.
Dialysis patients: consult prescribing information for specific recommendations.
## Contraindications
* Hypersensitivity to tab-genixim, other cephalosporins, or any component of the formulation.
* History of severe hypersensitivity reaction (e.g., anaphylaxis) to penicillin or other beta-lactam antibiotics.
## Adverse Effects
* **Common:** Diarrhea, nausea, vomiting, abdominal pain.
* **Serious:** *Clostridioides difficile*-associated diarrhea (CDAD), hypersensitivity reactions (including anaphylaxis), Stevens-Johnson syndrome, toxic epidermal necrolysis, seizures (rare, particularly with renal impairment and high doses).
## Key Drug Interactions
* **Probenecid:** May increase and prolong serum concentrations of tab-genixim.
* **Oral Contraceptives:** Efficacy may be reduced. Advise alternative or additional non-hormonal contraception.
## Monitoring
* Monitor for signs and symptoms of bacterial infection; clinical improvement should be observed within 2-3 days of initiating therapy.
* Monitor for signs of hypersensitivity reactions.
* Monitor for diarrhea; if *C. difficile*-associated diarrhea is suspected, discontinue tab-genixim and institute appropriate therapy.
* Renal function should be assessed periodically in patients with known or suspected renal impairment.
## Clinical Pearls
* Tab-genixim has broad-spectrum activity against many Gram-positive and Gram-negative pathogens.
* Ensure gastric decontamination is not performed if a rapid diagnostic test for *C. difficile* is to be performed, as this can lead to false-negative results.
* Should be taken with or without food.
