# Tradjenta (linagliptin)
## Overview
Linagliptin is an oral dipeptidyl peptidase-4 (DPP-4) inhibitor used to improve glycemic control in adults with type 2 diabetes mellitus. It increases incretin levels, leading to enhanced insulin secretion and reduced glucagon secretion in a glucose-dependent manner.
## Primary Indications
Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
## Adult Dosing
The usual dose is 5 mg orally once daily.
## Pediatric Dosing
Dosing has not been established in pediatric patients.
## Dose Adjustments
No dose adjustment is necessary for patients with renal impairment or hepatic impairment.
## Contraindications
* History of serious hypersensitivity reaction to linagliptin or any excipient in the formulation.
* Use with a strong CYP3A4 inducer (e.g., rifampin, carbamazepine, phenytoin, St. John's wort) may reduce linagliptin exposure and efficacy.
## Adverse Effects
* **Serious:** Angioedema, anaphylaxis, severe skin reactions (e.g., Stevens-Johnson syndrome, rash, urticaria), pancreatitis, severe and disabling arthralgia.
* **Common:** Nasopharyngitis, diarrhea.
## Key Drug Interactions
* **CYP3A4 Inducers:** Rifampin, carbamazepine, phenytoin, and St. John's wort may decrease linagliptin efficacy. Monitor glycemic control closely and consider alternative antihyperglycemic agents.
* **Other Antidiabetic Agents:** Increased risk of hypoglycemia when used in combination with sulfonylureas or insulin. Dose reduction of the sulfonylurea or insulin may be considered.
## Monitoring
* **Primary:** Monitor glycemic control (HbA1c, fasting plasma glucose, postprandial glucose).
* **Secondary:** Monitor for signs and symptoms of hypersensitivity reactions, pancreatitis, and severe skin reactions.
## Clinical Pearls
* Linagliptin is not intended for the treatment of type 1 diabetes mellitus or diabetic ketoacidosis.
* Due to its minimal renal and hepatic excretion, linagliptin is a preferred option in patients with significant renal or hepatic impairment.
* Patients should be advised to report any signs of allergic reaction or severe upper abdominal pain immediately to their healthcare provider.
