# Tebenipenem-Pivoxil
## Overview
Tebenipenem-pivoxil is an oral carbapenem antibiotic. It is a prodrug that is hydrolyzed to tebenipenem, the active form.
## Primary Indications
Tebenipenem-pivoxil is indicated for the treatment of acute bacterial exacerbations of chronic bronchitis (AABECB) in adults.
## Adult Dosing
* **AABECB:** 500 mg orally every 8 hours for 7 days.
* Maximum daily dose: 1500 mg.
## Pediatric Dosing
The safety and efficacy of tebenipenem-pivoxil in pediatric patients have not been established.
## Dose Adjustments
* **Renal Impairment:**
* Mild (CrCl 60-89 mL/min): No adjustment needed.
* Moderate (CrCl 30-59 mL/min): No adjustment needed.
* Severe (CrCl <30 mL/min) or End-Stage Renal Disease (ESRD): Efficacy and safety data are insufficient to provide specific dosing recommendations. Dose adjustment may be necessary, but specific guidance depends on local protocols and clinical judgment.
## Contraindications
* Hypersensitivity to tebenipenem-pivoxil, other carbapenem antibiotics, or any component of the formulation.
* Patients with a history of severe hypersensitivity to beta-lactam antibiotics.
## Adverse Effects
* **Common:** Diarrhea, nausea, vomiting, rash, abdominal pain, headache.
* **Serious:** Clostridioides difficile-associated diarrhea (CDAD), hypersensitivity reactions (including anaphylaxis), seizures (rare, typically associated with higher doses or renal impairment).
## Key Drug Interactions
* **Valproic Acid:** Tebenipenem-pivoxil may decrease valproic acid concentrations, potentially leading to loss of seizure control. If co-administration is necessary, close monitoring of valproic acid levels and seizure activity is recommended.
* **Probenecid:** May increase tebenipenem plasma concentrations.
* **Antidiarrheal agents:** Use with caution, particularly those that inhibit bowel motility, due to potential increased risk of CDAD.
## Monitoring
* Monitor for signs and symptoms of hypersensitivity reactions.
* Monitor for signs and symptoms of diarrhea, especially if severe or persistent, and consider testing for *C. difficile*.
* Monitor renal function, particularly in patients with pre-existing renal impairment or those receiving concomitant nephrotoxic agents.
* If co-administered with valproic acid, monitor valproic acid serum concentrations and seizure activity.
## Clinical Pearls
* Tebenipenem-pivoxil should be taken with or without food.
* It is important to complete the full course of therapy to ensure eradication of the infection, even if symptoms improve.
* Due to the potential for cross-reactivity, use with caution in patients with penicillin or cephalosporin allergies.
