# Tradjenta (linagliptin)
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor used to improve glycemic control in adults with type 2 diabetes mellitus. It works by increasing incretin levels, which leads to a decrease in glucagon levels and an increase in insulin secretion in a glucose-dependent manner.
## Primary Indications
Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus. It can be used as monotherapy or in combination with other antihyperglycemic agents, including metformin, sulfonylureas, and insulin.
## Adult Dosing
The recommended dose is 5 mg orally once daily.
## Pediatric Dosing
Dosing for pediatric patients has not been established.
## Dose Adjustments
No dose adjustment is required for patients with renal or hepatic impairment.
## Contraindications
* Known hypersensitivity to linagliptin or any excipients in the formulation.
* History of angioedema with DPP-4 inhibitors.
## Adverse Effects
Common adverse effects include nasopharyngitis and diarrhea. Rare but serious adverse effects include:
* Acute Pancreatitis: Symptoms include severe, persistent abdominal pain, which may radiate to the back with or without vomiting.
* Hypersensitivity Reactions: Including rash, urticaria, and angioedema.
* Severe and Disabling Arthralgia (joint pain).
* Hypoglycemia: Risk is increased when used in combination with sulfonylureas or insulin.
## Key Drug Interactions
* **CYP3A4 Inducers (e.g., rifampin):** May decrease linagliptin exposure and efficacy. Concomitant use is not recommended.
* **Sulfonylureas/Insulin:** Increased risk of hypoglycemia. Consider a lower dose of sulfonylurea or insulin.
## Monitoring
* HbA1c: Monitor periodically to assess glycemic control.
* Renal function: While no dose adjustment is needed, monitor as clinically indicated.
* Signs and symptoms of pancreatitis and hypersensitivity reactions.
## Clinical Pearls
* Linagliptin is primarily eliminated unchanged by the liver and does not require dose adjustment in renal impairment, making it a convenient option for patients with varying degrees of kidney function.
* Patients should be educated on the signs and symptoms of pancreatitis and advised to seek medical attention if they occur.
* Advise patients to report any signs of angioedema immediately.
