# Tradename: Tradjenta
## Overview
Tradjenta (linagliptin) is an oral dipeptidyl peptidase-4 (DPP-4) inhibitor used to improve glycemic control in adults with type 2 diabetes mellitus.
## Primary Indications
Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
## Adult Dosing
The recommended dose is 5 mg orally once daily.
## Pediatric Dosing
There is no established dose for pediatric patients.
## Dose Adjustments
No dose adjustment is necessary for patients with hepatic or renal impairment.
## Contraindications
* Known severe hypersensitivity to linagliptin or any excipient in Tradjenta.
* History of angioedema related to previous DPP-4 inhibitor treatment.
## Adverse Effects
* **Common:** Nasopharyngitis, cough.
* **Serious:** Hypersensitivity reactions (including anaphylaxis, angioedema, exfoliative dermatitis), severe and disabling arthralgia, bullous pemphigoid, acute pancreatitis.
## Key Drug Interactions
* **CYP3A4 Inducers:** Concurrent use of strong CYP3A4 inducers (e.g., rifampin, carbamazepine, phenytoin, phenobarbital) may decrease linagliptin exposure and efficacy. Consider alternative agents or increased monitoring.
## Monitoring
* **Glycemic Control:** Monitor HbA1c and blood glucose levels regularly.
* **Pancreatitis:** Assess for symptoms of pancreatitis (persistent severe abdominal pain, sometimes radiating to the back, with or without vomiting).
* **Hypersensitivity:** Monitor for signs and symptoms of hypersensitivity reactions.
## Clinical Pearls
* Linagliptin is unique among DPP-4 inhibitors as it is primarily eliminated unchanged by the liver, not requiring dose adjustments for renal or hepatic impairment.
* Risk of hypoglycemia is low when linagliptin is used as monotherapy. However, if used in combination with a sulfonylurea or insulin, dose reduction of the other agent may be necessary.
* Report cases of severe joint pain potentially indicative of arthralgia and bullous pemphigoid.
