# Tradjenta (linagliptin)
## Overview
Linagliptin is an oral dipeptidyl peptidase-4 (DPP-4) inhibitor used to improve glycemic control in adults with type 2 diabetes mellitus. It works by increasing incretin levels, which reduces glucagon secretion, improves insulin sensitivity, and slows gastric emptying.
## Primary Indications
Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
## Adult Dosing
The recommended dose of linagliptin is 5 mg orally once daily.
## Pediatric Dosing
Dosing for pediatric patients has not been established.
## Dose Adjustments
No dose adjustment is required for patients with renal impairment or hepatic impairment.
## Contraindications
* Hypersensitivity to linagliptin or any excipients in the formulation.
## Adverse Effects
* **Common:** Nasopharyngitis, diarrhea, cough.
* **Serious:** Hypoglycemia (especially when used in combination with sulfonylureas or insulin), severe and disabling arthralgia (joint pain), angioedema, Stevens-Johnson syndrome, pancreatitis.
## Key Drug Interactions
* **CYP3A4 Inducers (e.g., rifampin, carbamazepine, phenytoin, St. John's wort):** May decrease linagliptin exposure and efficacy. Concurrent use is not recommended.
* **Sulfonylureas and Insulin:** Increased risk of hypoglycemia. Dose reduction of the sulfonylurea or insulin may be considered.
## Monitoring
* **Blood Glucose:** Monitor glycemic control regularly.
* **Renal Function:** Baseline and periodic monitoring is not required due to lack of dose adjustment based on renal function.
* **Pancreatic Enzymes:** Monitor for signs and symptoms of pancreatitis (e.g., persistent severe abdominal pain, with or without vomiting).
* **Signs of Hypersensitivity/Angioedema:** Monitor for signs and symptoms.
## Clinical Pearls
* Linagliptin is primarily eliminated unchanged via the bile and gut, and to a lesser extent, via the kidneys, hence no dose adjustment is needed in renal or hepatic impairment.
* It has no known drug interactions mediated by CYP450 enzymes.
* Consider the increased risk of hypoglycemia when used with insulin or sulfonylureas.
* Educate patients about the signs and symptoms of pancreatitis and severe allergic reactions.
