# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone concentrations, stimulating glucose-dependent insulin release and decreasing glucagon levels. It is unique among DPP-4 inhibitors for its primary biliary excretion rather than renal clearance.
## Primary Indications
Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
## Adult Dosing
* **Standard Dose:** 5 mg orally once daily.
* **Maximum Dose:** 5 mg daily.
## Pediatric Dosing
Safety and effectiveness in pediatric patients (under 18 years) have not been established. Use is generally not recommended.
## Dose Adjustments
* **Renal Impairment:** No dosage adjustment indicated.
* **Hepatic Impairment:** No dosage adjustment indicated.
* **Geriatric:** No dosage adjustment indicated; however, monitor for volume depletion/hypotension if combined with other agents (e.g., sulfonylureas or insulin).
## Contraindications
* History of a serious hypersensitivity reaction to linagliptin (e.g., anaphylaxis, angioedema, exfoliative skin conditions).
## Adverse Effects
* **Common:** Nasopharyngitis.
* **Serious:** Acute pancreatitis (rare), severe and disabling arthralgia (post-marketing reports), bullous pemphigoid, and hypersensitivity reactions.
* **Hypoglycemia:** Low risk as monotherapy; risk increases when combined with insulin or insulin secretagogues (e.g., sulfonylureas).
## Key Drug Interactions
* **CYP3A4/P-gp Inducers:** Strong inducers (e.g., rifampin) may decrease linagliptin efficacy. If co-administration is necessary, consider increasing monitoring.
* **Insulin/Secretagogues:** May require dose reduction of the secretagogue to minimize hypoglycemia risk.
## Monitoring
* **Glycemic Control:** HbA1c (typically every 3–6 months); daily blood glucose logs per patient needs.
* **Clinical:** Monitor for signs/symptoms of pancreatitis (persistent severe abdominal pain) and worsening skin lesions (bullous pemphigoid).
## Clinical Pearls
* **Renal Safety:** Linagliptin is the only DPP-4 inhibitor that does not require dose adjustments for any degree of renal impairment.
* **Administration:** May be taken with or without food.
* **Pancreatitis:** Discontinue immediately if pancreatitis is suspected.
* **Joint Pain:** Consider discontinuation if a patient experiences severe, persistent joint pain.
