# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone concentrations, stimulating glucose-dependent insulin release and decreasing glucagon levels. It is unique among DPP-4 inhibitors for its primarily non-renal route of elimination.
## Primary Indications
Type 2 diabetes mellitus (T2DM) as an adjunct to diet and exercise to improve glycemic control.
## Adult Dosing
Standard dose: 5 mg orally once daily.
Maximum dose: 5 mg daily.
## Pediatric Dosing
Safety and effectiveness have not been established in patients younger than 18 years of age.
## Dose Adjustments
No dose adjustment is required for patients with renal impairment (including end-stage renal disease) or hepatic impairment.
## Contraindications
* Hypersensitivity to linagliptin (e.g., anaphylaxis, angioedema, urticaria, or bronchial hyperreactivity).
* History of pancreatitis (use with caution).
## Adverse Effects
* **Common:** Nasopharyngitis, increased blood uric acid.
* **Serious:** Acute pancreatitis (including fatal cases), severe and disabling arthralgia, bullous pemphigoid, and hypersensitivity reactions.
## Key Drug Interactions
* **Strong CYP3A4/P-gp Inducers (e.g., rifampin, carbamazepine, phenytoin):** May decrease the efficacy of linagliptin. If co-administration is necessary, monitor glycemic control closely.
* **Insulin/Insulin Secretagogues:** When combined with insulin or sulfonylureas, the risk of hypoglycemia may increase; a lower dose of the insulin or secretagogue may be required.
## Monitoring
* HbA1c (typically every 3–6 months).
* Monitor for signs and symptoms of pancreatitis (persistent, severe abdominal pain radiating to the back) and bullous pemphigoid (blistering or erosions).
## Clinical Pearls
* **Biliary Excretion:** Linagliptin is primarily excreted unchanged via the bile; therefore, renal dose adjustments are not necessary—a significant clinical advantage in patients with chronic kidney disease.
* **Administration:** May be taken with or without food.
* **Not for Type 1:** Linagliptin is not indicated for the treatment of type 1 diabetes or diabetic ketoacidosis.
