# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone concentrations, stimulating glucose-dependent insulin release and decreasing glucagon levels. It is unique among DPP-4 inhibitors for its primary biliary excretion rather than renal.
## Primary Indications
Type 2 diabetes mellitus (T2DM) to improve glycemic control as an adjunct to diet and exercise.
## Adult Dosing
5 mg orally once daily. May be taken with or without food.
## Pediatric Dosing
Safety and efficacy have not been established in pediatric patients (under 18 years of age). Use is not recommended.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment required.
* **Hepatic Impairment:** No dose adjustment required.
## Contraindications
* History of serious hypersensitivity reactions (e.g., anaphylaxis, angioedema, exfoliative skin conditions) to linagliptin.
* Current use is not indicated for the treatment of Type 1 diabetes or diabetic ketoacidosis.
## Adverse Effects
* **Common:** Nasopharyngitis, increased risk of hypoglycemia when used with insulin or insulin secretagogues (e.g., sulfonylureas).
* **Serious:** Acute pancreatitis (discontinue if suspected), severe and disabling arthralgia, bullous pemphigoid, and hypersensitivity reactions.
## Key Drug Interactions
* **Strong CYP3A4/P-gp Inducers (e.g., rifampin):** May decrease linagliptin efficacy. If co-administration is necessary, monitor glycemic control closely.
* **Insulin/Insulin Secretagogues:** Risk of hypoglycemia is increased; consider dose reduction of the secretagogue or insulin.
## Monitoring
* **HbA1c:** Perform at baseline and every 3 to 6 months.
* **Renal Function:** Not required due to primary biliary excretion, but recommended as part of routine diabetes care.
* **Clinical:** Monitor for signs/symptoms of pancreatitis (persistent, severe abdominal pain) and bullous pemphigoid (blistering or erosions).
## Clinical Pearls
* Linagliptin is the only DPP-4 inhibitor that does not require dose adjustments for renal failure, making it a favorable choice in patients with chronic kidney disease (CKD).
* Does not typically cause weight gain.
* If a dose is missed, instruct the patient to take it as soon as remembered, but do not take two doses on the same day.
