# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin levels (GLP-1 and GIP), which enhance insulin secretion and decrease glucagon levels in a glucose-dependent manner. Unlike other DPP-4 inhibitors, it is primarily excreted via the biliary/fecal route.
## Primary Indications
Type 2 diabetes mellitus (T2DM) as an adjunct to diet and exercise to improve glycemic control.
## Adult Dosing
Standard dose: 5 mg orally once daily.
Maximum dose: 5 mg daily.
## Pediatric Dosing
Safety and efficacy have not been established in patients younger than 18 years.
## Dose Adjustments
No dose adjustment is required for renal impairment (even in end-stage renal disease) or hepatic impairment.
## Contraindications
* History of hypersensitivity reaction (e.g., anaphylaxis, angioedema, urticaria) to linagliptin.
* Type 1 diabetes or treatment of diabetic ketoacidosis.
## Adverse Effects
* Common: Nasopharyngitis, hypoglycemia (when used with insulin or sulfonylureas).
* Serious: Pancreatitis (including fatal cases), severe and disabling arthralgia, bullous pemphigoid, hypersensitivity reactions.
## Key Drug Interactions
* Strong CYP3A4 or P-glycoprotein inducers (e.g., rifampin, carbamazepine, phenytoin): May decrease linagliptin efficacy. If possible, consider an alternative agent if these drugs must be used.
* Insulin or Sulfonylureas: Increased risk of hypoglycemia; consider dose reduction of the secretagogue.
## Monitoring
* HbA1c (typically every 3–6 months).
* Renal function (baseline and periodic, though dose change is not required).
* Monitor for signs/symptoms of pancreatitis (persistent, severe abdominal pain).
* Monitor for signs of bullous pemphigoid (blistering or erosions).
## Clinical Pearls
* Linagliptin is unique among gliptins as it does not require renal dose adjustment, making it a favorable choice in patients with chronic kidney disease.
* Linagliptin can be taken with or without food.
* If a dose is missed, take it as soon as remembered, but do not take two doses on the same day.
* Physicians should counsel patients on the importance of maintaining adequate hydration and reporting severe abdominal pain immediately.
