# Tabgenixim (Cefixime)
## Overview
Tabgenixim is a brand-name formulation of **Cefixime**, a third-generation cephalosporin antibiotic. It provides bactericidal activity by inhibiting bacterial cell wall synthesis. It is stable against many beta-lactamases.
## Primary Indications
* Uncomplicated urinary tract infections (UTIs)
* Otitis media
* Pharyngitis and tonsillitis
* Acute bronchitis and acute exacerbations of chronic bronchitis
* Uncomplicated gonorrhea (cervical/urethral/rectal)
## Adult Dosing
* **General infections:** 400 mg orally once daily or 200 mg every 12 hours.
* **Uncomplicated gonorrhea:** 800 mg orally as a single dose.
* **Maximum dose:** 400 mg per day for standard infections; 800 mg for single-dose gonorrhea.
## Pediatric Dosing
* **Weight-based:** 8 mg/kg/day orally, administered as a single dose or divided into 4 mg/kg every 12 hours.
* **Otitis Media:** 8 mg/kg/day once daily or 4 mg/kg every 12 hours.
* **Note:** If weight exceeds 50 kg, use adult dosing.
## Dose Adjustments
* **Renal Impairment:** If CrCl <60 mL/min, reduce dose or extend intervals.
* CrCl 21–60 mL/min: Administer 75% of the standard dose.
* CrCl <20 mL/min: Administer 50% of the standard dose.
* **Hepatic Impairment:** No specific adjustment required.
## Contraindications
* Known hypersensitivity to cefixime or other cephalosporins.
* History of severe hypersensitivity (anaphylaxis) to penicillins or other beta-lactams (cross-reactivity risk).
## Adverse Effects
* **Common:** Diarrhea (frequent), abdominal pain, nausea, dyspepsia.
* **Serious:** *Clostridioides difficile*-associated diarrhea, hypersensitivity reactions (rash, urticaria), seizures (rare, mostly in renal impairment), and hematologic abnormalities (leukopenia, thrombocytopenia).
## Key Drug Interactions
* **Anticoagulants:** May enhance the effects of warfarin (monitor INR).
* **Probenecid:** Increases serum levels of cefixime by inhibiting renal excretion.
* **Vaccines:** May decrease the efficacy of live typhoid vaccine.
## Monitoring
* Monitor for signs of hypersensitivity reactions during initial doses.
* Observe for persistent diarrhea, fever, or abdominal cramping (*C. diff* risk).
* Monitor renal function in patients with pre-existing impairment.
## Clinical Pearls
* Cefixime has poor activity against *Staphylococcus aureus* (it is not effective for MRSA or MSSA).
* Diarrhea is significantly more common with this drug than with many other oral cephalosporins; taking with food slightly improves tolerability.
* Suspensions should not be refrigerated; store at room temperature and discard after 14 days.
