# Linagliptin
## Overview
Linagliptin is a dipeptidyl peptidase-4 (DPP-4) inhibitor that increases incretin hormone concentrations, stimulating glucose-dependent insulin release and decreasing glucagon levels. It is unique among DPP-4 inhibitors as its primary route of elimination is biliary/fecal, requiring no renal dose adjustments.
## Primary Indications
Type 2 diabetes mellitus as an adjunct to diet and exercise to improve glycemic control.
## Adult Dosing
Standard dose: 5 mg orally once daily.
Maximum dose: 5 mg daily.
## Pediatric Dosing
Safety and efficacy have not been established in patients younger than 18 years. Use is generally not recommended in pediatric populations.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment required regardless of GFR or end-stage renal disease.
* **Hepatic Impairment:** No dose adjustment required; however, caution is advised in severe hepatic impairment due to limited clinical experience.
## Contraindications
* Hypersensitivity to linagliptin or any component of the formulation.
* History of serious hypersensitivity reactions (e.g., anaphylaxis, angioedema, exfoliative skin conditions) to DPP-4 inhibitors.
## Adverse Effects
* **Common:** Nasopharyngitis, hypoglycemia (when used with sulfonylureas or insulin).
* **Serious:** Acute pancreatitis, bullous pemphigoid, severe joint pain (arthralgia, may occur months after starting), and hypersensitivity reactions.
## Key Drug Interactions
* **Strong CYP3A4/P-gp Inducers (e.g., rifampin):** May decrease linagliptin efficacy; increase monitoring of glycemic control if co-administration is unavoidable.
* **Insulin/Insulin Secretagogues:** Risk of hypoglycemia is increased. Consider dose reduction of the secretagogue or insulin when initiating linagliptin.
## Monitoring
* **Hemoglobin A1c:** Typically every 3–6 months.
* **Blood Glucose:** Periodic self-monitoring per clinical guidelines.
* **Signs/Symptoms of Pancreatitis:** Persistent, severe abdominal pain that may radiate to the back, with or without vomiting.
* **Skin Integrity:** Monitor for blistering or erosions suggestive of bullous pemphigoid.
## Clinical Pearls
* Linagliptin is weight-neutral and does not cause significant weight gain.
* It does not require dose adjustments based on renal function, making it a preferred DPP-4 inhibitor in chronic kidney disease patients.
* If pancreatitis is suspected, discontinue immediately and do not restart.
* Not indicated for the treatment of Type 1 diabetes or diabetic ketoacidosis.
